0056076609 2016-09-02 UrbanProduction Wells No Violations I inspcted this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger lift system in a small, chain-link enclosure which was locked and secure. Fittings and connections at the wellhead were secure and free from signs of leakage. The tank batery is located 250 feet northeast of the well at the edge of the school grounds, in a separate, chain-link enclosure. The enclosure gates were locked and secure. Secondary containment was intact and free of signs of spills or leaks. Fittings and connections at the battery were secure, but the load-out valves were not locked. I also noted the presence of 60 feet of 3-inch steel tubing laying on the outside of the battery. I contacted Jeff Schonauer, contract pumper for JM Smail, and advised about the need for locks on the load-out valves, and the need to remove the unused piping. Jeff indicated that the piping had previously been utilized as an extension for the oil truck in the winter, but would have it removed since it is unused.

The tank battery is equipped with two 100 barrel production tanks, one 1500 gallon poly brine tank, a mechanical separator and the meter run. Equipment spacing is correct. The tank battery is fenced and the dike is in good repair. Identification signs are posted on the tank battery and the well. The man doors are locked. The lease road is stable and constructed out of durable material.
0777986797 2007-06-21 Drill / Deepen / Reopen No Violations Operator was repairing pit liner at the time of inspection. Leak was located, operator was pumping pit down, so that they would be able to reline the pit. Operator was waiting on cement to set. Inspected drill bit, bit was new and in good repair. Operator called at 4:45pm BOP test was at 5:45 unable to attend BOP test.

The well and battery are located on the north portion of the grounds of the East Lake Middle School and Lakeshore Blvd. The wellhead mechanical plunger lift is located in a small chain-link enclosure which is locked. The fitting and connections at the wellhead are secure and free from signs of leakage or problems. The tank battery which is comprised of 2 100-barrel steel tanks (1 production, 1 brine) and mechanical separator. Interior spacing is compliant. Load-outs are secure. The gate is locked and secure. Fittings and connections are free from signs of leakage or problems. Areas surrounding the well and battery are snow-covered. I did not observe any signs of impact from the well or problems with runoff.
1182274352 2007-06-19 Drill / Deepen / Reopen No Violations Operator is constructing pits. During excavation operator hit a green drain tile, was attempting to locate a replacement at the time of inspection. Operator also hit the remains of a foundation of a building while digging the pits. Location of the pits is in fill material. Operator needs to get a permit on site before any more site construction is completed. All permit conditions need to be followed.
1182526871 2007-06-21 Drill / Deepen / Reopen No Violations On site for surface cement. Operator set CND last night. Operator informed me that the pit linear had a leak, the fluid was flowing threw the fill material that the pits were constructed on and into a creek. Fluid was fresh water with Gel that was used to treat the fill material. Operator was pumping down pit into tanks, to locate the leak. Operator is going to draw down the pit, reline the pit. Surface cement went well, good returns.

The spacing is in accordance with the rule, and the valving and mechanicals are in good repair. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a mechanical lift device and is located in a separate gated 6-foot chainlink enclosure. The gates on the enclosures are locked. The tank battery dike is adequate. No violations noted.

At the time of this inspection, I found the following: Fittings and connections at the well and battery were secure and free from leakage. Enclosure gates and load-line valves were locked. The well was in active production. Production reports were current. The short access road to the well and battery from the asphalt parking area was in good condition, with a light snow covering. The well had recently changed ownership and the signage had not yet been updated. Im notifying the new owner about the need for updated signage.
