Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0221871246 2007-10-19 Drill / Deepen / Reopen No Violations well being fraced
0263410894 2008-10-14 Preliminary Restoration No Violations
0536389723 2017-04-10 UrbanProduction Wells No Violations I inspcted this producing well as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspction, I had noted that the load-line valves were not secure with locks as required. At the time of this inspection, I observed the following: the access road to thewell and battery, which is comprised of a gravel-paved road that enters from the existing asphalt-paved road utilized by the industrial facility and travels 30 eet to the battery, was in good condition with no drainage problems or wash-outs. The separate enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance and safety defficienbcies; the load-line valves remained unsecure and require locks, the ID signage had not been updated to reflect the recent change in ownership. I contacted John Magnussen with David Oil and advised him about the continuing defficiency. He indicated that the items are being worked on as they are able. I contacted Richard Osborne Sr. with David Oil Co., and as of the date of this report, I have not received a response.

0681146423 2016-02-09 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. The well and associated battery are located in an otherwise open, grassy area on the east side of the commercial building at this location. The well and battery are accessed via a short, unpaved road that connects to the existing asphalt-paved driveway. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system in a board-on-board enclosure. Fittings and connections at the wellhead were secure. The 8.625-inch casing is valved at surface but not gauged. The battery is comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank and separator in separate board-on-board enclosures. Enclosure gates were locked and secure. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surround both the well and battery and did not observe any signs of impact relative to the production activity. Signage was present. I noted the following safety issues: The load-line valves were not locked. The owner must remedy the following by 04/08/2016: secure the load-line valves with locks. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
0860080482 2010-04-14 Production Wells No Violations No violations were found at the time.
0963284225 2017-12-11 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves were not locked and secure. At the time of this inspection, I observed the following: the load line valves were not locked and the ID signage remains unchanged to reflect the new ownership transfer that occurred in 2016. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
1293156199 2012-12-05 UrbanProduction Wells No Violations The unit is comprised of a two-tank battery (1 210 and 1 140-barrel) and a wellhead with mechanical plunger lift, and mechanical separator, each in separate wood board-on-board enclosures. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. The tank battery secondary containment is adequate. I did not observe any leaks, spills, or other signs of contamination. The gates are locked.
1312213949 2011-07-28 UrbanProduction Wells Violations Noted Dike, spacing, ID in compliance. Vent hatches are installed and closed. Feedlines are bullplugged. No locks on gates, lightning arrestor and charcoal not installed.
1313048185 2013-10-09 UrbanProduction Wells No Violations The producing unit consists of a wellhead and aboveground storage tank (AST) battery in separate board-on-board enclosures. The wellhead is a mechanical plunger lift. The battery is comprised of 1 210-barrel and 1 140-barrel steels tank and separator. Secondary containment, spacing, and signage is good. Process safety equipment (spark arrestor, carbon filter, and low-pressure relief valve) are present. Gates are locked. The ID placard is located on the inside of the battery and not visible. I will contact John D. Oil about moving the placard to the outside. No problems otherwise noted.
1850341377 2019-02-14 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 681146423, regarding locks needing installed on the load-line valves, and identification updated to reflect new ownership. At the time of this inspection, the well equipped with plunger lift assembly was not in production. The well was fenced with a locked door. The tank battery production tanks still needed locks installed on the tank valves. The tank battery consisted of one 210 BBL tank, one 100 BBL tank and a mechanical separator. The diked tank battery was secured with a locked fence but the identification located on the fence still needed updated to reflect current owner information. Lease access to the well and tank battery was stable.

2079209791 2007-09-25 Drill / Deepen / Reopen No Violations location made ready for rig.
2105933150 2015-04-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located in a green space portion of a property currently occupied by a light-industrial facility. The well is comprised of a mechanical plunger-lift system in a board-on-board enclosure which is locked. Fittings and connections at the wellhead are secure. The battery is comprised of one 210-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings and connections are secure. The secondary containment is intact and dry. Load-outs are secure. The areas surrounding the well and battery consist mainly of turf grass which is well maintained and free from impact or erosional deficits.
