0092292381 2016-12-02 Orphan No Violations I inspected this well location as part of evaluation for possible Orphan Well Program. I met with Orphan Well Inspector Kevin Perry and a representative of the home owner. At the time of this inspection, construction of a single-family home on the parcel on which the well is located. Work on the house structure and earthwork around the house was on-going. The well was located near the SE corner of the house, 18 feet +/- from the east exterior wall of the house. The well was comprised of a 9-inch steel casing riser with reducer swage to a 2-inch Balon valve which was closed. There was no leakage of gas observed at the wellhead. There was no other surface production equipment noted. The homeowners, through their representative have expressed interest in pursuing assistance from the Orphan Well Program for plugging and abandonment. Kevin Perry will be handling the submission.
1034967055 2018-04-13 Orphan No Violations I inspected this well location pursuant to Complaint #5817. A report of gas odor had been made to the Mentor Fire Department, in the vicinity of this well and the nearby Schultz #1. Upon my inspection, I found the well to be secure with a closure swage and no indications of leakage. John Magnussen from Rockefeller oil was at the location of the Schultz #1 making the well ready for anticipated plugging and abandonment. The well was shut-in and the pump jack assembly had been removed from the well. I did not encounter any leaking gas or odors. I contacted the Mentor Fire Department and reported my findings.
1484147513 2016-12-02 Orphan No Violations I was on location with inspector Tompkins to investigate the possible orphan well on the property. The location is currently excavated for the construction of a house. The well is constructed with 8.625 casing and 6.5 casing. The 6.5 casing is reduced with a swage to 2. There is a 2 balon valve installed on the well that is currently in the closed position. The guage on the well is reading 25 psi. There is no sign of a domestic supply running from the well. The well will be submitted to the orphan well program for consideration.
