The company has erected a tank battery consisting of a total of 8 tanks of 400 barrel capacity located inside of a polymer lined metal containment dike area. Proper well identification is posted.
0166255400 2017-07-17 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with erosion control and vegetation established in the affected area of the well pad. There are (4) producing wells on the pad with the productin valves open and the wells selling to gas processing units with (4) units on location. Supporting the gas processing units are (8) steel production tanks that are 400 barrel cepacity with the tanks all situated inside a polyment lined steel containment dike area. Proper well identification is posted.

The well pad has a earthen berm perimeter and the well pad surface is of stone construction. The lease road is gravel construction and in good repair.

There are (4) producing wells, (4) gas processing units (GPU), (2) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 100 psi.
1427268637 2015-03-25 Casing String Witnessed No Violations On site inspection for a 3:00 am call out to witness the conductor cement job found the following. The said well was spudded on 03/24/2015 by Rocky Mountain Drillings top hole rig with a 32 x 100 conductor borehole being drilled and 100 of 26 weld connection conductor being set. Express Energy Cement service cemented the conductor using 330 sacks of class A-2% calcium .125 pps lcm -100% open hole excess cement displaced with 55 bbl of fresh water with good cement returns witnessed to surface.
1430051339 2015-04-25 Casing String Witnessed No Violations On site to witness running&cementing of surface casing string,Patterson Rig 342 drilled to a depth of 1255 (17.5hole) and then ran 1255 of 13.375 casing and Halliburton cemented it in place with 1270 sacks of cement which weighed 15.6# per gal with a yield of 1.21 and they had 87 bbl of good cement returns and cement stayed up good.
1430827638 2015-05-05 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found Patterson Drilling rig 342 on location with the 12.250 intermediate recently drilled to a depth of 7767 and 7747 of 9.625 was ran and set in the wellbore, Halliburton Cement service cemented the casing using a 930 sack ExpandaCem lead cement and a 33 sack HalCem tail cement with 16 bbls of the Tergovis spacer returned to surface and a estimated top of cement at 154.
1434971160 2015-06-22 Plug / Plug Back No Violations On 06/20/2015 it was reported by the company man on site that while administering the Formation Integrity test out from under the 9.625 casing string in the 8.750 open hole well bore section it was discovered that the formation would only stand a 13.4# mud weight before breaking down below the 9.625 casing shoe. The open hole total measured depth was 7,635 and a Class H -147 sack 16.35 # plug was set through the 5 19.5 # drill string ran to bottom. The company then waited 48 hours on the cement to cure before going back in to begin to re drill and retest the well bore. My Supervisor Brent Bear was updated on the situation.
1434988932 2015-06-22 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Patterson Drilling rig # 342 on location drilling the 8.750 production borehole at a current measured depth of 13,600 in the horizontal well bore section in the directional azimuth of 332.59 degrees with a borehole inclination of 91.14 degrees and true vertical depth of 8,337.
