0589338522 | 2016-04-05 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the area affected by the well site construction meets ORC 1509.072 with erosion control and vegetation established, the well pad perimeter is enclosed in a with a 8 fence and locked gate. Addressing the well head - on this day it was observed that the well is equipped with a 2.875tubing head with the production valves in the open position, equipment also present on the well head is (2) manual shut down valve and (2) General Electric Hydraulic emergency shut down actuators with a primary well pad shut down button located entry point of the location outside the well pad perimeter gate. Also on location is 2 Dehydration gas processing units and 2 mechanical fluid separator units with 3 -400 barrel production tanks present inside of a polymer lined metal containment dike area. |
0676500018 | 2015-08-28 | Drill / Deepen / Reopen | No Violations | My inspection today finds the drilling rig, H&P 531, encountered an influx of gas while drilling at 15,025. As a result, the rig gained ten barrels of diesel based drilling mud in their pits. A peak of 5200 units of gas were detected in the mud solution. I witnessed a consistent flare of approximately 15 feet in height. The rig was circulating on the well and building the weight of the mud. The azimuth direction of the well is 143.23 degrees. |
0779637565 | 2015-07-06 | Casing String Witnessed | No Violations | I am onsite to witness the conductor cementing operations. Precision Rat Hole drilled 113 of 36 borehole and ran 113 of 118 pound weld casing. Universal cemented the casing in place with Type 1 cement. I witnessed 25 barrels of cement circulate back to surface. Eighteen inches of cement was left in the cellar to provide a liquid tight seal. |
0858143992 | 2018-02-27 | Production Wells | No Violations | Status check inspection found the well pad lease road has signage posted and is gated and locked. The well pad has an 8 chain link fence that is also gated and locked with signage posted at the entrance also. There is also a emergency shut down device at the well pad entrance. On the well pad there is 2 active wells present with annular pressure monitoring in place on each well and the well tender reporting no abnormal pressures recorded or encountered. Each well head is configured with a casing flow cross and (2) outlet valves in place however no casing flow lines are attached. Attached to the tubing heads are (2) manual and (2) remotely controlled emergency shut down valves. Each of the 2wells on location has proper well identification posted. Also on location is a large natural gas compressor followed by a gas processing unit and 5 gas dehydration units, one supporting each well on the pad. At the far left hand side of the well pad is the production fluid storage area consisting of a total of (4) tanks that are steel construction and situated inside a polymer lined containment area. |
1074112195 | 2015-07-30 | Casing String Witnessed | No Violations | I am onsite to witness the surface cementing operations. At the time of my arrival, H&P rig 531 had already ran 879 of J- |
1439848116 | 2015-08-17 | Casing String Witnessed | No Violations | 08/17/2015 - 16:00 pm- On site inspection to witness the mine void casing cement job. H&P Rig 531 is on location and has drilled a 24 mine void to a depth of 930 with the mine void encountered at 827 to 840. The company then ran 885 of 87.5#/ppf J-55 Casing with a float shoe and collar ran along with a inflatable packer set at 684. Schulumberger Cement service was on location to cement the casing using a 132 sack class A 15.6# 1.20 yield mix to cement the bottom section. Upon landing of the plug for the bottom section the pressure was increased to verify casing integrity and then the internal casing pressure was increased to inflate the packer, once the packer seal was verified the DV tool was opened and the top section cemented using 1,150 sacks of class A 15.6# 1.20 yield 50% open hole excess cement with the cement weight verified @ 15.6#/ppg. |
