0511094406 | 2017-10-10 | Drill / Deepen / Reopen | No Violations | On the day of my inspection, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The Orion Rig #14 had drilled to a depth of 4,253 feet with an azimuth of 218 degrees and an inclination of 15.74 degrees at 4,200 feet. The 13 3/8 inch diameter surface casing was set at a depth of 1,822 feet. |

1320846986 | 2019-04-18 | Production Wells | No Violations | On this day, I found the pad gated with signage at the entrance. There are (2) producing wells , (2) gas processing units, (2) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 125 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. There is an erosional concern on the northern slope at the pad entrance. I contacted Ryan Plute with Equinor to inform him the issue. The preliminary restoration will be addressed at a later date. |
1420377303 | 2018-03-06 | Completion Testing | No Violations | Status check inspection found the well drilling and completion process complete with the well head master valve in place and annular pressure monitoring installed with gauges present. Crews are on site installing the production related equipment. The polymer containment and flow back tanks from the stimulation process are still on site. The Preliminary Restoration will need followed up on when more adequate vegetation is established. The well pad perimeter berm and final grade slope appears to be stable. The well pad is gravel and will need final grading at some future point as the completion process ends. |
