I passed out my ODNR card and a list of all of the Inspectors, Supervisors, and Central Office staff numbers. Matt advised that he would begin drilling sometime Saturday.
1342533977 2012-07-16 Drill / Deepen / Reopen No Violations I arrived today to check on drilling status. I found that the Tin Horn has been installed along with the Conductor. This Conductor cement job was rescheduled 4 times. There is 100 ft of 20 inch Conductor set and cemented with 252 sks of Class A cement. This was drilled by a Top Hole machine and is now ready for a Drilling rig to move on.
1343934934 2011-07-31 Drill / Deepen / Reopen Violations Noted I am on site today to find out the drilling depth and see when the 13 3/8 casing will be installed and cemented. During my inspection of Nomac Drilling rig # 39 I found that they have already installed the 13 3/8 casing. I ask Matt why I had not been notified of the operation and he replied that I had . He said that he notified me 48 hours ago when he started drilling. I advised him that I made it clear that we needed to be contacted before the installation of any casing and that we witnessed the cementing as well.

1513227146 2016-02-16 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. All wells on location are using the plunger lift system as the method of production. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

10 400 bbl. Production tanks 3 Producing wells 3 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Horizontal heater-treater. Equipment present on the location that is not associated with the production equipment consists of: 3 18 Wheelers with trailers 1 track hoe 1 Short bed semi 1 dump truck 1 skid steer 1 power plant 2 pickup trucks

8 small trailers 1 40 foot box trailer 1 16 foot box trailer 3 fuel tanks 35 joints of 9 5/8 inch diameter casing. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

2141163894 2012-07-11 Drill / Deepen / Reopen No Violations DNR Inspector Mark Moreland and I were on site to check the status of Express Energy Service. Our inspection revealed that they have started drilling the hole for the tin horn (cellar). The rig consultant (Gene) advised that they should finish drilling the conductor hole and run casing later this evening. He also stated that if everything went according to plan, then they should be ready to cement the conductor casing sometime tomorrow morning. I advised Gene that Inspector Moreland was taking over the regulatory duties in Harrison County so he would need to contact him about cementing the conductor. Our inspection revealed no violations at this time.
