0044273961 2002-07-09 Production Wells No Violations Inspection revealed drilling pit liner has fallen below fluid level, and fluids has been allowed to over flow the drilling pit. There is a kill area located at the North East corner of the drilling pit that is approximately 110 down grade and 34 wide. I took three (3) photos of the kill area. I stopped at H & D office but no one was in. OWNER NEEDS TO DO THE FOLLOWING. ( 1 ) Remove all fluids and close the drilling pits immediately. ( 2) Remove contaminated soils and replace with soil capable of supporting vegetation. I found no Identification at the well head. (Information on the pumping unit is incorrect.

0814061862 2015-07-22 Production Wells Violations Noted Inspect well as part of a production well status check. The well was equipped with a pumping unit and motor. Identification was posted at the well. This well producess into a shared tank battery with P#1993, P#2871, and P#2295. The tank battery consists of (2) 100 barrel tanks and separator inside an earthen dike. Identification was posted at tank battery. The tank battery is currently in violation for causing pollution and contamination (OAC 1501:9-1-07) and failure to keep dike free of brine water (OAC 1501:9-9-05 (A)(9). 0868038853 2016-04-01 Production Wells No Violations This inspection was conducted as a compliance notice follow-up. I arrived to the location at 10:15 AM to witness clean-up operations regarding the release. Division Emergency Operations Coordinator Vikki Bunting was on location when I arrived. H & D Drilling was in progress of clean-up. A back hoe was used to remove soil from the ditch and the area around the flow line. The well will be shut-in until a new line is installed. During the inspection it was revealed that the 100 barrel brine tank associated with the tank battery for this well was leaking brine from the bottom of the tank. The leak varied from at least a drop per second to a constant stream. A dike was observed around the tank. Mr. Donaker was instructed to have tank emptied immediately and to not use tank until it is repaired or replaced. During the inspection Wilsons Water Hauling LLC. UIC#25628 arrived and pumped out the brine tank. The flow line clean-up was completed. It was decided to leave booms in the stream to catch any residual run off of oil from future rain events. The booms will be monitored and left in place until it is decided they can be removed. Time of inspection: 10:15 AM.

1982446164 2016-03-31 Production Wells Violations Noted I received a phone call at 1:48 PM from Licking Soil & Water Zach Bollheimer regarding an 1.5 steel flow line leak that allowed oil to enter a ditch. I arrived to the location at 3:15 PM. The leak occurred along the northern side of Frampton Road at this approximate location: N 40.21630 W -82.25375. A pumper for H & D Drilling was present when I arrived. The inspection revealed oil had entered the ditch and traveled approximately 75 feet where it entered a culvert that crosses Frampton Road. The culvert discharges just on the other side of Frampton Rd. where it then enters a stream. Oil nor a sheen could be observed in the stream. Well owner Harold Donaker and OEPA OSC Trevor Irwin arrived that evening. Mr. Donaker applied diapers and absorbent to the ditch. OSC Trevor Irwin placed two booms in the stream. It was estimated that the amount of oil observed in the ditch was less than half a barrel. Once the oil was contained it was agreed to allow H & D Drilling to start clean-up the following day. Note: Division Emergency Operations Manager Trisha Cartmell was notified of release. Time of inspection: 3:15 PM. Weather: 65*F, Overcast
