Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0303660934 2015-01-26 Production Wells No Violations This inspection was conducted as a status check. The rods and tubing have been removed. The well will be placed into production as a swab well. A 100 barrel tank has been brought in. The well had a 4 to 2 swage with a 2 valve in the closed position. Identification will need to be posted once tank battery is built and well is in production. Time of inspection: 1:30 PM. Weather: 27 degrees F, 4 snow
0381376239 2017-09-15 Final Restoration No Violations The inspection revealed that the 100 barrel tank has been removed and grass seed was applied. The well location meets ORC 1509.072 requirements. Time of inspection: 1:15 PM. Weather: 71*F, Partly cloudy
0423871367 2017-02-13 Plug / Plug Back No Violations I returned to this well today to witness the plugging of it. The rig crew was able to work the 5.5 inch casing free after pulling 6 joints. We then set a 250 feet Big Lime plug at this point. From there the rig crew pulled the 5.5 inch casing up the hole to set the bottom of the surface plug at the 7 inch casing since the Berea was behind pipe. We pumped a 200 foot plug at the bottom of the surface. After that plug was set we pulled the 5.5 inch diameter casing up to set a 200 foot surface plug. The cement held at the surface. The well is now plugged.
0845703305 2017-08-03 Final Restoration No Violations The purpose of this inspection is to document a phone conversation with Mr. Jim Aslanides of MFC Drilling Inc. regarding the final restoration. Mr. Aslanides was informed that the restoration is almost due to be complete as it has been approximately 6 months since the well has been plugged. Mr. Aslanides informed be that he would have someone look at it and work on getting it completed. Time of phone call: 1:17 PM.
1374968115 2014-11-04 Production Wells No Violations This inspection was conducted as a production well status check. The well consisted of rods and tubing but no pumping unit or flow line. No tank battery or identification.
1405673296 2017-06-13 Final Restoration No Violations This inspection was conducted as a status check. The well has been plugged. The final restoration is due to be completed by 8/13/2017. The inspection revealed that a 100 barrel tank and metal flow line has not been removed as of this date. A compliance notice will be issued if restoration is not complete by 8/13/2017. A phone call was placed to MFC Drilling President Mr. Jim Aslanides to inform him of what all needs done. Mr. Aslanides was made aware of the time frame to have it completed by. Time of inspection: 2:00 PM. Weather: 85*F, Sunny
1411905024 2015-04-15 Production Wells No Violations Inspect well. No change in status. A phone call was placed to Jeff Harmon with MFC Drilling. Mr. Harmon stated that this well is scheduled to be plugged this year. Time of inspection: 2:30 PM. Weather: 64*F, Sunny
1584217845 2017-08-18 Final Restoration No Violations During the inspection the well location was observed. The final restoration has not been completed. It appears that the flow line has been removed but a 100 barrel tank was still at the well location. It will be suggested to spread some grass seed/straw on some areas that vegetation is sparse. The company will be granted an extension of 30 days. A compliance notice will be issued on or after 9/18/2017 if the restoration is not complete. MFC Drilling President Jim Aslanides was contacted via phone conversation and was advised of the due date. Time of inspection: 10:00 AM. Weather: 79*F, Partly cloudy
1922074639 2017-02-10 Plug / Plug Back No Violations I was on location today to witness the plugging of this well. MFC Drilling has there service rig in on the well to plug it. Today I witnessed the bottom plug being set through the 5.5 inch diameter casing. We bullheaded the bottom plug because the well was dry instead of pumping the plug through the tubing. Once the plug was set the rig crew ran the ripping tools in the well to rip the 5.5 inch diameter casing. The rig crew was able to find the telegraph at 1670 feet indicating the pipe was free at this spot. The rig crew was able to rip the casing at 1670 feet but the pipe was hung at this point. The rig crew worked the pipe up and down to free it up in the hole. They ended up shutting down at 4 pm. They will be working on freeing up the casing Monday morning.
2111274998 2014-11-06 Administrative Inspection No Violations A phone call was placed to Jeff Harmon with MFC Drilling regarding the status of this well. Mr. Harmon stated that this well is scheduled to be plugged next year.
