1480523400 2016-11-28 Final Restoration No Violations I conducted a final restoration inspection on this well. The well is located in Industrial Mineral Permit IM-192. The well is being mined through. The tank Battery and lease road were shared with the Raymond Johnson #2 (151-24790) which is still in production. The final restoration is passed.
1481823427 2016-07-05 Plug / Plug Back No Violations I was on location with DOGRM Inspectors Murphy and Frye to witness plugging operations for this Clinton Sandstone well. All plugging operations were conducted by Chipco, LLC (Rig #5) and J. Hawk Water Hauling. Tim McConaha (Enervest) was present for all operations. Chipco blew the well down and began rigging up for plugging operations. Chipco rigged up to the 4.5 production casing and mixed and pumped 6.3 barrels of Class A cement slurry. The slurry was bullheaded with 62 barrels of water and locked up at 300psi. The plug was sat on for one (1) hour and Chipco broke off and the well was dead. The remaining three (3) plugs were set to isolate the Big Lime, the Berea Sandstone/Ohio Shale interval, and the glacial deposits. See plugging report for more details on this job. No violations.
1820944312 2016-07-06 Plug / Plug Back No Violations I was on location to check the top plug in this well that was plugged and abandoned yesterday (7/5/16). My inspection found the top of the plug 15 below grade and there was no indication of leakage or gassing. The 8.625 surface casing will be lowered (cut off) by 85 as surface mining operations progress around this plugged and abandoned well. I met with the mine operator and he has the ID plate to attach to the top of the casing when lowering is completed. Note: A 300 plug was placed in the top of this well to compensate for the lowering of the casing.
