0298598540 2018-07-18 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 4.5-inch diameter casing with a casing head, 2.875-inch diameter intermediate casing with a tubing head, 1.5-inch diameter tubing, and rods. There was no pumping unit attached but the platform with wooded blocks remained beside the well. All valves appeared to be in the open position. The tubing was equipped with a cross tee fitting with one of the two sides having an open valve with no line attached and open to the atmosphere. While using the Industrial Scientific Ventis Pro 4 gas detector I recorded zero readings for the lower explosive limit or Hydrogen sulfide. The well was situated within forest like surroundings with thick underbrush consuming the well. A lease road to the well could not be identified. There was not any identification located in a conspicuous place on or near the well. No production reports have been submitted to ODNR since the year 2001.
0595743088 2016-04-07 Production Wells No Violations My inspection today finds the well is equipped with 4.5 casing, 2.875 intermediate casing, 1.5 tubing, and rods. There is no pumping unit on site. Production valves are open at the well head. The tank battery consists of three 210 bbl tanks in a dike. There is also a separator but no meter. The well will be referred to the orphan well program. The well does not appear to be currently used for domestic gas. I attempted to make contact with the home owner, but no one was home.
