The Chipco service rig attempted to run a packer on tubing into the hole to pump on the well and test for the leak. The rig was unable to get the packer past 142. It was decided that the well would be swabbed down to remove the fluid and an Appalachian logging truck will be brought out in the morning to run a down hole camera and identify the issues with the casing. |
0208048689 | 2014-06-20 | Completion Testing | No Violations | I was contacted by Jeff Foley of Enervest (814-493-3893) and informed that the 4.5 production casing failed during the hydraulic fracturing of this well. The series of events were as follows: A safety meeting was held and the fracturing manifold pressure tested to 3,600 psi. Universal Pumping attempted to stimulate the well. They achieved formation break at 2,548 psi, and pumped 12 bbl and shut down 5 minutes for acid to work. While establishing pump rate at 19 bpm at 2,500 psi, fluid began to flow from the 8 5/8 annulus, indicating possible 4.5 casing issues. The job was shut down and frac crews were rigged down and moved from location. A temporary 100bbl production tank was brought on location and diked to provide a place to flow the frac fluids from the well into. All returned fluids were contained in the tank. Enervest shut down the operation for the weekend and will be bringing in a Chipco LLC service rig to begin to evaluate the issue with the well. There will be no further stimulation of this well until the issue has been identified and a plan of action has been approved by ODNR. |

Pep Drilling drilled a 7 78/ hole to 5277 and circulated the well clean before running the casing. 150sks of 50/50 2% gel, 10% salt cement was mixed and pumped. It was displaced with 12bbls of acid and 69bbls of fresh water. The plug was landed and pressured up 500psi over land pressure and held for 5 minutes with no pressure bleed off. Pep Drilling will now move on to the Kolm #2 well. |

Inspector Bob Roush was on location to witness the pumping of 150sks of cement to squeeze and cover the hole. Chipco service rig is on location currently drilling out the cement left in the casing. They expect to be drilling out the cement until tomorrow when further steps will be determined. |

Centralizers were on joints 1, 4, 8, 12, 16 and the top. In total 14bbls of cement were returned to surface. They will be testing the blow out preventer in 12 hours. |

The 8 5/8 annulus was filled with water and fluid began to fill the 4.5 casing indicating that the casing issue is below this depth. They will evaluate their options and work deeper in the hole to find the issue. |

The rig has had a gas monitor installed on the well. There are no violations at this time. |

The rig has received a load of used production casing. I have checked the casing tickets to confirm the casing has been pressure tested to 4400psi by Buckeye Supply. Some of the used casing which passed pressure testing requirements was rusted, scaled and pitted. The more rusted, scaled and pitted casing will be inserted first so that it will be encased in cement. Cementing this casing is planned for tomorrow morning. |
