During my inspection the well was equipped with a production tree style wellhead and a plunger lift system with production valves in the open position. The production equipment consisted of eighteen 400 barrel tanks within a lined steel containment, six gas production units within a lined steel containment, one vent scrubber, two compressors, and two combustors. The pipeline tie in is located on the north side of the pad. The production records are current and the identification was located at the wellhead.
0133634714 2013-06-26 Drill / Deepen / Reopen No Violations My inspection today finds the well was struck by lightning at 7:00 pm last night. The lightning burnt out a module in the top drive. The top drive has been repaired and the rig is currently drilling at a depth of 6350. A review of the site indicates that the operator can remove the super silt fence around the location.
1025027733 2018-06-12 Production Wells No Violations The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 202 pounds per square inch, 370 pounds per square inch on the production casing, 0 pounds per square inch on the 9.625 inch diameter intermediate casing, and 170 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of eighteen 400 barrel tanks, six gas production units, and two combustors. The identification was located at the wellhead.

1458879711 2013-06-11 Casing String Witnessed No Violations I am onsite to witness surface casing cementing operations. Patterson rig 343 ran 565 of 18.625 casing. Superior Well Service cemented the casing in place with 470 bbl of class A cement. 176 bbl of cement circulated to surface.
1475985360 2013-06-19 Production Wells No Violations My inspection today finds that the drilling rig is currently drilling 12.25 hole at a depth of 2411. I reviewed the BOP test results.

There are six wells on this pad; 3412124408, 3412124409, 3412124407, 3412124471, 3412124472, and 3412124428. During my inspection the well was equipped with a production tree style wellhead. The production valves are open. The production equipment consists of six gas processing units, eighteen 400 barrel tanks within containment, and two flare stacks. The production records are current, identification is located on the wellhead, and there are no violations present.

1658479373 2017-06-07 Production Wells No Violations The purpose of my inspection was a status check on a producing well. The pad is named Rich and there are a total of six wells on this pad. During my inspection the well was equipped with a production tree style wellhead and a plunger lift system with production valves in the open position. Gauges present read 285 pounds per square inch on the casing and 209 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consists of eighteen 400 barrel tanks within a lined steel containment, six gas processing units within a lined steel containment, two compressor, and two combustors. The pipeline tie in is located on the north side of the pad. The production records are current, the identification was located at the wellhead and gas production unit, and there were no violations present.
1678703212 2018-11-01 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 150 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 6- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. There was a small gas leak below the B section of the well head, I contacted Zac Cornish an Environmental Technician with Antero and made him aware of the problem. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
1680264343 2014-03-31 Production Wells No Violations My inspection finds well is in production. The tank battery consists of eighteen 400 bbl tanks that are shared with three other wells. The identification is posted as required. The well site is being prepared for a drilling rig.
1906748746 2015-04-02 Preliminary Restoration No Violations I am accompanied Inspector David Ball today. Our inspection finds the well appears capable of production with the production valves open. The tank battery is comprised of eighteen 400 barrel tanks set in a liquid tight plastic lined steel containment. A separate containment area holds five production units. The production equipment is shared with four other wells on the pad. The well site meets the preliminary restoration requirements.
1923670241 2013-11-21 Completion Testing No Violations My inspection today finds the service rig has drilled out the kill plug at 5001 and is running tubing to the first plug.
