0506968124 2018-11-26 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 0 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site near located to the south of the pad. Identification tags were present, and production was up to date.
0685864706 2016-05-18 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The production valves are open. The tank battery consists of eight 400 barrel tanks set in a plastic lined steel containment system. Antero personnel Zac Cornish and Josh Horn are on location. I am accompanied by a number of division staff.

1157932356 2014-11-05 Preliminary Restoration No Violations My inspection today finds the well is equipped with tubing and is produced by open flow through a choke. The tank battery is comprised of eight 400 barrel tanks located in a steel framed and plastic lined containment. The well site meets preliminary restoration requirements.
1380040473 2013-09-24 Production Wells No Violations My inspection today finds Express Drilling drilled a 32 inch hole to a depth of 111. 26 casing was ran to 109. Allied cemented the casing in place with 336 sks of class A cement. I witnessed 10 bbl of cement returns to surface. Twenty bbl of cement is still in the casing due to the job being stopped because of rising pressures.
1473677442 2013-09-27 Casing String Witnessed No Violations My inspection today finds the rig crew ran 335 of casing. Allied cemented the casing in place with class A cement. I witnessed 80 bbl cement returns to surface.
1489154345 2017-03-10 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of three gas production units, eight 400 barrel tanks, two compressors, and two combustors. Identification was present and production records were current.
1514416990 2017-12-27 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Pressure monitoring was present. The production equipment consisted of eight 400 barrel tanks within steel line containment, three gas production units, one vapor recovery tower, two compressors, one vent scrubber, and two combustors. The identification was located at the wellhead.
1712140707 2013-10-03 Drill / Deepen / Reopen No Violations My inspection today finds the 13.625 casing is parted at 286. The drilling superintendent is attempting to locate a casing patch. Once the patch is in place they will grout the backside of the casing.
