0465059932 2006-01-10 Production Wells No Violations Status check inspection - no violations at this time. I contacted Lynn Strazi and requested that he place a chain link fence around the well head. The surrounding area continues to be developed as a single family residential allotment.
1048816564 2003-03-26 Production Wells No Violations The tank battery has been dismantled and impacted soil was removed on 3/24. Supr. Cheslock witnessed a portion of the removal.
1105366299 2016-01-26 UrbanProduction Wells No Violations The Rubber City Sand and Gravel # 1 well was turned into line 7/12/1985. A search of the RBDMS data base indicated that the Operator was current in the reporting of production volumes. The well was constructed of 478 of 8.625 surface casing ( 194 sacks ), 4401 of 4.5 production casing ( 140 sacks ) and 4176 of 1.5 tubing. The well was operated as a plunger lift well. The well head was fenced but not locked. I did not find any leaks or contamination. The well head is located in a residential development that did not exist when the well was drilled. The well head did not have a I. D. sign, but I will request the Operator to place one at the well head and place a lock on the fence surrounding the well. The road to the tank battery was graveled and afforded adequate access. The tank battery was fenced and locked. The required I. D. signage was in place and legible. The tank battery consisted of one (1) 100 barrel oil tank, one (1) 33 barrel poly brine tank, one ( 1) 2408 mechanical separator and a DOM / EOG gas measurement station. The load line was plugged with a flat plug but it was not tight. No violations found during this inspection.
1327530335 2003-03-06 Production Wells Violations Noted During a status check inspection, the 210 bbl stock tank was found to have holes and was seeping. There was also no identification at the wellhead. The three Rubber City wells have a common tank battery with three 210 bbl tanks, one 100 bbl tank and three horizontal separators. I contacted Dan Long (330-866-3558), the pumper for Cutter and informed him of the problems.
1734254283 2003-05-19 Production Wells No Violations The old 210 bbl tank has been reset. The site will be monitored on a regular basis and if the tank leaks again, it will need to be replaced. The pollution and identification violations have beed corrected.
