Review of records in RBDMS indicates the last reported commercial production was in 2006 for this well. The well is of plunger lift construction. The well is shut in with all valves on the wellhead closed. The well is fenced but the man door was not locked. The tank battery has a 100 BBL production tank, a 1500 gallon steel brine tank and a mechanical separator. The production tank valve has a bull plug. There was a small oil stain observed under the bull plug. The tank battery is fenced but not locked. The signage on the tank battery was illegible. The lease road is stable and drivable. The following items need to be completed by:
1) Vegetation removed from around the well head and from inside the tank battery (1-28-15) 2) Identification sign posted (1-12-15) 3) Well shall be plugged (6-27-15) |
1472222143 | 2016-08-11 | UrbanProduction Wells | No Violations | I conducted an inspection on this well. The well is equipped with a plunger lift unit and the valves on the wellhead are closed. The well is fenced and the man door on the fence was not locked. There is no identification at the well. The tank battery consisted of one, 100 BBL production tank, one, 50 BBL brine tank and a mechanical separator. The 100 BBL production tank is seeping at the base underneath the tank valve. There is visible staining under the tank valve. The tank battery is fenced but the man door was not locked. The identification sign was illegible and has not been updated. Vegetation has been maintained at the tank battery and well. Access to the wellsite was stable. A plugging permit was issued for this well on 10/8/2015. Issues remaining to restore compliance include:

1525451889 | 2018-05-01 | UrbanProduction Wells | No Violations | I conducted a follow up inspection to assess the level of compliance with Compliance Notice # 914162647 dated 12/29/2014. My inspection revealed that the tank battery was removed by PPG Inc. in June 2017. PPG contracted with Gonzoil Inc. to remove the tank battery and dispose of the fluids and equipment properly. PPG considered the tank battery a liability. The stone tank pad still remained in place. The well was stabilized with 50 barrels of brine in order to change out any leaking fixtures per PPG instructions. My inspection today showed the well was completed as a plunger lift well. The valves on the wellhead were in the closed position. I did not identify any leaks or contamination. The wellhead was secured with a locked chain link fence. The I. D. signage was posted and legible. Two (2) of the issues denoted in the previous Compliance Notice have been remedied. Those were I. D. signage ( 1501:9-05 [ E 1 ] ) and locks ( 1501:9-05 [ E 3 ] ). The other two (2) issues remain unresolved, General Safety and Well shall be Plugged and as such the Compliance Notice ( # 914162647 ) remains in effect. |

My inspection found Chiefs Order 2016-391 for Failure to Maintain Liability Insurance has not been complied with. The well is not plugged as requested by the Compliance Notice issued on 12-29-14. 1624597220 | 2017-09-21 | UrbanProduction Wells | No Violations | The Collins # S-8 was located within the PPG plant compound in Barberton. Access was gain through the Guard Station located at the end of 16th Street south of Wooster Ave. I inspected the well as a follow to Notice of Violation ( # 914162647 ) issued 12/29/2014. I met with Hadley Stamm ( 724.325.5809 ) and, after the in-house safety orientation, proceeded to the wells location. Ms. Stamm informed me that the tank battery for the Collins S-8 well had been removed and the well shut in by a Contractor ( Gonzoil - Doug Gonzales @ 330.571.0140 ). Ms. Hadley said, this work was performed by PPG amid concerns for further contamination at the site . The area is under an U S EPA compliance order for remediation of the Lime Lakes on premises. Per Mr. Doug Gonzales the tank battery was completely dismantled, steamed out and removed, fluids disposed in Class II injection wells. All the tank pad was removed, the area graded and seeded. The vegetation was well established during my inspection. There was not any surface expression of previous Oil and Gas activity at the former tank battery site. The wellhead was fenced and locked. A non - compliant I D sign was attached to the fencing. The wellhead was disconnected from the flow line and the flow line was purged with fresh water. Bull plugs were placed in the two (2) inch side casing valves and the annular ( 8.625 X 4.5 ) port valve. The Demco valve, on the 1.5 tubing, was in the closed position. The last reported production volumes was for the year 2016 reporting year. The well was not plugged or in production. The Notice of Violation ( # 914162647 ) has not been complied with at the time of this inspection. |
