Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0000PL4142 Preliminary Restoration No Violations Preliminary Restoration done information from Data Point
0073912560 2014-03-14 UrbanProduction Wells Violations Noted I inspected this well and tank battery on 3/14/2014. The tank battery consists of a 100 Bbl. Stock tank, 50 Bbl. Poly brine and vertical separator. The dike is insufficent on the North side to contain the fluids within the diked area. I found a unplugged drain pipe in the dike that would allow any fluids to breach the dike. The access road was graveled and passable. All the required identification was in place and legible. The proper spacing between the tank and separator was observed. All loadlines were secured and locked. The well consisted of a Churchill Model 40 pumping unit with the associated pumping well plumbing and was leak free. The electrical box was locked as required. In order to bring the Kuhar #1 into compliance the Owner shall remove the dike drain and rebuild the North side of the tank battery dike to a height sufficent to contain stored fluids. Owner shall use an impervious material to reconstruct the dike. No other violations were identified during this inspection.
0109635598 2006-04-28 Production Wells Violations Noted Status check inspection found a leaking stuffing box, no identification on the tank battery and insufficient secondary containment. I called George Strawn and informed him of the deficiencies at this site. He stated that the problems will be corrected.
0607929708 2019-03-28 Production Wells No Violations My inspection found the well is equipped with a pumpjack. There is a 2017 Annual Production Report in the wells file. The tank battery consists of steel production tank, a plastic brine tank, a vertical separator and a meter. The well is legibly identified. No compliance issues are cited at this time.
1412175432 2014-06-05 Production Wells No Violations The dike surrounding the tank battery of the Kuhar # 1 has been brought into compliance. No other violations were observed during my inspection.
2041982641 2006-05-12 Production Wells Violations Noted The impacted area has been remediated, the tank battery has been sand blasted and painted and the containment dike has been extended. The NOV has been corrected.
