1443445660 2013-06-06 UrbanProduction Wells No Violations I inspected this well site on 6/6/2013 as a follow up to Complaint # 4342 of a neighboring Landowner concerning oil in the Dominion/ East Ohio Gas lines servicing a residence at 3126 Clubside Drive. The Johnson Unit # 1 well is operated by an electrified pump jack. The tank battery consists of a 210 barrel oil tank, 30 barrel poly brine tank and a horizontal 2408 mechanical separator with a positive shut off drip. I spoke with the complainant, by phone, and relayed my conservation with Mr. Tim Levengood ( Industry Standard Supplies, Inc. ). Mr. Levengood told me that the positive shut off drip had failed to operate as designed and allowed oil to pass into Dominion/ East Ohio Gas distribution lines. The offending drip tank has been fixed and now is fucntional. No violation identified at this time.

1971344387 2011-03-04 Production Wells No Violations Status check inspection found that the pump jack was installed and pumping. The fence has been installed around the tank battery but not around the wellhead. I called Tim Levengood and told him to shut the well down until all of the fence was installed. There is also a question as to the legality of one of the leases in this unit. Columbus has requested but not yet received additional documentation from ISSI.
