The mechanicals are in good condition and the TB dike is present and intact. Tank spacing is good. The wellhead is enclosed within a 6-foot, wood slat enclosure. The wellhead is fitted with a plunger-lift system. I observed gas leaking or venting from the wellhead. I advised Cuyahoga County Inspector Ray Wallace about leaking gas and he indicated that he would follow up with the well owner.

The wellhead and battery are located behind a single-family residence at the referenced address. Access to the well is via the shared residential driveway. The wellhead plunger lift and battery are in separate board-on-board enclosures which are locked. Fittings and connections are secure and free from signs of leakage or problems. The load-outs are secure, plugged, and locked. Process safety gear is present. Signage is in place and current. At the time of this inspection there are 15 to 20 inches of snow cover in the area which obscures some surficial elements from observation.
1629779645 2016-02-04 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: Ingress and egress to and from the well area is afforded by the shared house driveway with short, unpaved section at the battery. A wellhead with mechanical plunger lift system is located in a board-on-board enclosure in the otherwise open portion of the back yard. The well enclosure was locked and secure. Fittings and connections at the wellhead were secure and free from leakage. The 8.625-inch casing annulus was exposed and fitted with pressure-relief fitting. Areas around the wellhead consist of turf grass which was free from signs of stress or impact. The tank battery is located 50 feet south of the wellhead within the same grassy and wooded yard area. The battery is comprised of one 100-barrel, steel production tank, one 50-barrel poly brine tank. At the time of this inspection, the access gate to the battery was locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. Tank lids were closed. The load-outs are located remotely from the battery near the road, and were plugged and locked. Screening was present surrounding the battery and wellhead and comprised of deciduous shrubs that were bare owing to the season, but in otherwise good condition. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation. I did observe the following maintenance issue: the production tank lid is missing the closure wing-nut. This needs replaced to effect a proper seal. I notified the owner about the need to replace the missing wing nut.

The well and battery are located at the back portion of a single-family residence. At the time of this inspection, I found the following: a wellhead plunger-lift unit and associated battery, accessed via the shared driveway, with a short, gravel-paved extension to the battery. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The well was in active production. The chart recorder was active. Production reports were current. Bonding and insurance was in place and current. I noted that the board-on-board enclosure at the wellhead was dismantled. I contacted Brooks Yarborough with Baay Oil and he reported that service work on this well was pending and the enclosure would be replaced when that was completed.
