I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity. |
1223044646 | 2008-09-30 | Production Wells | No Violations | Inspector Lowder received a call reporting a wellhead release at the Valley Vista well on Wallings Road in North Royalton. The initial reports were that something had failed and rods had been pushed out of the hole by escaping oil and natural gas. The problem was first noticed by teachers working the nearby playground who reported that they heard a loud noise followed by dust around the well. The school administration notified Cutter who responded with equipment and personnel. Upon arrival they found that five full rods and several pup rods along with the polished rod were out of the hole both inside and outside of the fence and the well was spraying oil/water. The rods were bent and damaged by both the incident and during initial response actions. Cutter shut the remaining valves on the wellhead and a high pressure plug was placed in the tubing. The dust was apparently an oil/water spray, which coated the pumping unit, a majority of the board-on-board fence surrounding the well and was carried by wind to the east and impacted trees and ground cover approximately 100 downwind. The groundcover consisted primarily of recently spread grass seed and straw mulch. When I arrived at approximately 15:20, Cutter personnel were wiping down the pumping unit and fence and removing the impacted mulch. Cutter will have Intrepid Well Service on site in the next few days to pull the downhole pump and tubing (if necessary) and try to determine the cause of the incident. I met on site with Rick Simmers, the North Royalton Fire Chief and the North Royalton Assistant Superintendent and discussed the initial response and future actions that would be taken. |

There is an unpaved access road which travels south from the school parking area to a multi-purpose learning area and then to the well. At the time of this inspection, there are no wash-outs or erosional deficits along the access road. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The tank battery is comprised of a total of two 100-barrel steel production tanks, one 50-barrel poly brine tank, and mechanical separator. The entry doors are secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. The entry point signage is broken. Signage is present but lacking complete information. There is leakage at the stuffing box and oil residue within the wellhead secondary containment which needs cleaned up. An empty 55-gallon drum and chemical pump should be removed if not used. Carbon filters must be periodically evaluated for properly operation. In addition, the following safety issues were noted. Load lines are not locked. Brine is actively leaking from the 2-inch flange at the brine tank. Lightning arrestors are missing from the production tanks. The 8.625-inch casing is not exposed for monitoring as required. These safety issues must be remedied as soon as practicable or by 06/19/2015.
