At the time of this inspection, the flow-line had been repaired and the areas impacted effectively remediated and restored. Fittings and connections at the wellhead were secure and free from signs of leakage or problems.

The unit is back in production. The screening vegetation around the tank battery is mostly dead and will need re-planted. I will follow-up with Kleese to track the on-going restoration of the impacted area until completed.
1112700360 2013-02-19 Preliminary Restoration No Violations The wellhead consists of a pumpjack unit in an 6-foot fenced enclosure. The associated tank battery consists of 1 210-barrel (BBL) tank, 1 50 BBL tank, and a mechanical separator, also in a separate, locked enclosure. The spacing is in accordance with the rule. The tank battery dike is adequate. There is also a 50-BBL poly tank located at the wellhead, which has an earthen secondary containment. The dike drain is open. I contacted Matt Kleese and advised him about the open dike drain. There has been no production reports filed for this unit since 2010. I contacted Maryann at Kleese Dev. Assoc. She indicated that they may have forgotten to file the 2011 production report.
1377549931 2013-08-20 UrbanProduction Wells No Violations Got a call from the ODNR Uniontown office regarding a report of an oil release associated with this producing unit. Upon my arrival I observed crews from Kleese Development performing mitigation of a surficial coating of oil which had originated from a break in the sales line from this unit. Matt Kleese reported that the incident was reported at 1130 and Kleese crews were on site within the hour. Upon arrival, the Kleese crews shut-in the unit and relieved pressure from the line. They believe that the sales line became over-pressurized and ruptured. I estimate that less than one barrel of oil was released from the line. Also on site is a crew from the City of Cleveland Utilities Department HazMat team. I spoke with Mr. Juan Roman, a representative of the Hazmat Division, and he indicated that a groundskeeper witnessed the event and made the report. The property on which the well is located is owned by the City of Cleveland. He indicated that they were satisfied with the response by Kleese to the incident. I told him that I would be following up to confirm that the incident is appropriately mitigated and the proper repairs are made.

1432838646 2015-05-28 UrbanProduction Wells Violations Noted I inspected this well location following a phone notification from Kleese Development Assoc (KDA). That there had been a release of oil from the pumping unit. According to the report, a contract pumper noticed the leakage at some point during the day on 05/27/2015. Upon my arrival I spoke with Jim Adams from J Well (contract pumper). He indicated that an estimated 1 barrel or less of oil was release from a flow-line connection which failed near the pumping unit, within the containment. I observed a 30 ft. x 10 ft. area within the containment of free oil and oil-impacted ballast and soil. At the time of this inspection, crews from KDA are working on mitigating the areas impacted by the release and removing free oil with a vac truck. Impacted soils will be removed and disposed. The well has been shut-in during these activities which are on-going. In addition, an area of stressed vegetation that extends 75 feet downslope from the well is being mitigated. Clean-up and mitigation, including site restoration, must be completed within 30 days. Please contact Inspector Stephen Tompkins at (330) 907-4647 regarding this inspection.
1678596089 2017-09-28 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: There had been an uncontrolled release of oil impacting the area surrounding the well. At the time of this inspection, all impacted areas have been remediated and display good growth of vegetation. The well is in production, with secure fittings at the well and battery. Areas down-gradient of the well had shown some erosional impacts from surface drainage around the well, but the erosional deficits appear to have improved.

1935405940 2013-08-22 UrbanProduction Wells No Violations The pipe break has been repaired and the area of the break and subsequent impact has been partially regraded. Addtiional re-grading will be needed followed by re-seeding. I met with Frank Petrik, regulatory compliance manager for the Cleveland Department of Utilities who was on site. We talked to Jim Adams, contract pumper for Kleese who was also on site. Jim was returning the producing unit to operation. He indicated that the suspected cause of the line break was partailly due to a faulty valve at the Dominion metering station. Jim reported that Dominion has since fixed the valve and the unit is producing at normal operating pressure of 80 PSI. Jim indicated that final grading and seeding was pending. I observed an area of dead grass, approximately 500 square feet in area, beside the wellhead.
