Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1308061161 2011-06-09 Production Wells No Violations Request made by Nancy Wagner , she wanted well inspected because property is being sold. Well is a (GAS well No fluid ) located along the southern edge of drive way in a vault. No Violation found at time of inspection. Phone call made to Columbus to check Bonding and Insurance all which are current.

Well Is in Compliance.

1552201972 2016-11-23 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The well is located in the back of a single-family residence, in a sub-surface vault. At the time of this inspection, the well was comprised of 6-inch wellhead with reducer fitting to 2-inch supply connection and associated valve fittings and knock-out tanks. The vault was partially filled with water, obscuring much of the elements. I met with well owner Ron Gibosn at the location. I did not note any meter-detectable gas within the vault of ambient areas outside of the vault. Access to the well is via a 1/4-inch sheet metal lid, which was replaced and secure following inspection. The production reports are current, as well as bonding and insurance.
