0105482549 2017-01-18 Production Wells No Violations I returned to this well today to do a follow up inspection on it. At the time of my inspection the well is still sitting idle and incapable of production. Ralph Fetherolf the landowner of the well met me on location to show me documents from the courthouse that shows the lease has been broken on this well. During my field inspection today I called Elmer , he is the operator and owner of Victor McKenzie Drill Co. I talked with him to see when he was planning on plugging this well. He stated to me that he is sending in for the plugging permit for the well now. They were working on getting it complete. I asked him what time frame he was looking at to plug the well. Elmer then stated he would call me when he got the plugging permit and go from there. I will call him back next month to check on the status of it. I took new pictures of the well today during my inspection for my report.
0240700234 2018-07-03 Plug / Plug Back No Violations I was on location today to witness the plugging of this well. the rig crew removed the rods from the well but were unable to get the tubing pulled. The tubing is stuck at around 1800 feet with the stretch they were able to pull. They brought out a pump truck to see if we could pump through the tubing but it pressured right up. We will have to get the tubing cleaned out before we can pump a plug.

0310334317 2016-08-11 Administrative Inspection No Violations I called and left Elmer the owner and operator of Victor McKenzie Drill Co. a message to call me back in regards to this well today after my inspection. I have not been able to reach him and I have not gotten a phone call back.
0398403865 2016-08-11 Production Wells No Violations I arrived back at this well to do a follow-up inspection on it. I was unable to get down the lease road or find any opening to get to the lease road because of all the briars, thorn bushes, poison ivy and vegetation in general. The land owner was with me during my attempt and he told me he could clear a path with his tractor. I told him that would be great and I would be back in a few days.
0456466614 2016-08-16 Production Wells No Violations I returned to this well today to do a follow-up inspection on it. The land owner was nice enough to make a path for me to get to the well. There was no way to get to the well when I was here on 8-11-2016. I arrived at the well and found the well is over grown with vegetation. The valves are still shut in at the well making the well incapable of production. There is also no motor on the pumping unit to operate it. As before stated in my previous inspection report the land owner reminded me that the electric company had came out and disconnected the electric to the well. This well is still in violation with no attempt to get the well back in production. I will be issuing the company Victor McKenzie Drill Co. a Chiefs order on this well.
0513828607 2017-08-18 Production Wells No Violations I returned to this well today to follow-up on it. At this time the well is still in violation. The well is not capable of producing at this time. I made a phone call to his cell phone and did not get an answer so I called his office number. His wife or secretary answered and told me that she was sending out for the plugging permit soon. She stated that Elmer would be having surgery next Tuesday. After getting off of the phone with her I tried Elmers cell phone again and it went to voicemail. I left him a message with my phone number to call me back.
0523547416 2018-01-09 Production Wells No Violations I returned to this well today to do a follow- up inspection on it. There has been no change in the status of the well. I still have not received a plugging permit at this time for the well. I did talk with the landowner while I was on location and he was talking about wanting to get the production tanks off of his property. I let him know that the State was working on getting this well plugged.
0557840946 2018-05-09 Production Wells No Violations I was back on location today to follow-up with Elmer Gibson on the time frame of plugging this well. I was able to reach Elmer today by phone and he does have Altier Brothers lined up to plug the well. He said Benny Altier has already been out here to the well and he will be doing the plug job. I tried to call Benny Altier to see when they would be getting here but he was not in so I left a message for him to call me. Benny called me the next morning. He will be in contact with the landowner to see how he wants them to bring the rig in to plug the well.
0682881614 2018-07-11 Plug / Plug Back No Violations Today the rig crew worked on retrieving the 8 inch diameter casing. They had to rip the casing to get the pipe out. The rig crew found free pipe at 130 feet. Once the pipe was ripped the rig crew pulled out the casing. I was not on location today to watch them pull out the pipe. I was on another plug job. I received all information over the phone.
0739692652 2017-06-19 Production Wells No Violations I returned to this well today to do a follow-up inspection on it. The well is still idle and in violation at this time. I tried to contact Elmer to talk with him again about the plug report but his wife told me he is in the hospital at this time. He may have had a small stroke and will be in there a few more days. I will do a follow-up on this well again in a couple of weeks.

0898007660 2016-01-28 Production Wells Violations Noted I was on location to do a field inspection of this well. During the inspection of this well I found the well is not in production and is incapable of production. The valves are all shut in at the wellhead at this time. The well owner/operator shall submit a written plugging schedule to (Justin Smith) at (5880 Memory Road Zanesville, Ohio 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

The well is still idle and in violation. I tried to contact Elmer to see if he has sent in for the plugging permit yet but I did not get an answer. I left a message on his phone today regarding this well. I left my name and phone number for him to call me back.
1264210144 2017-04-19 Production Wells No Violations I returned to this well to do a follow-up inspection on it. During the time of my inspection the well was still in violation. The well is idle at this time with all of the valves shut at the wellhead. I contacted Elmer the owner of the company and he told me he has the plugging permit filled out he just has to round up the money for the plugging permit. I will talk to my supervisors to see what they want to do with this situation.
1264238124 2018-07-02 Plug / Plug Back No Violations I received a phone call today from Benny Altier to let me know the rig crew will be pulling the rods out of the well today then pulling the tubing out. The plan is to run the tubing back in the well to set the bottom plug.

Elmer stated that he is working with Altier Brothers to see when they can get in on the well to plug it. I told him to contact me as soon as they give him a time on will they will be there to plug it.
1371763184 2016-06-20 Production Wells No Violations I returned to this well to do a follow-up inspection on it. This well is still idle and in violation. I talked to the land owner of the well. He said the electric company came out and took down the meter box. They were contacted by Victor McKenzie Drill Company to do so because they were cancelling the service. No other actions have been taken on this well to get it in compliance.
1438239952 2018-07-10 Plug / Plug Back No Violations Today the rig crew pulled the rest of the 4.5 inch casing from the well then ran the 7 inch pre-cast plug down to 1000 feet. The rig crew then ran the tubing back in the well to 1000 feet to spot a 150 foot plug on top of the pre-cast plug. The tubing was then pulled from the well and the rig crew shut down for the day.
1499645208 2017-11-07 Production Wells No Violations I called Elmer today to talk to him about getting the plug permit for this well. He told me he would see if they sent in for it and if they didnt he would get it in. I told him that the compliance date to have this issue resolved has passed and the well needs to be plugged right away. Elmer said he would call me back after he finds out if it has been submitted or not.
1521722855 2018-03-22 Production Wells No Violations I returned to this well today to do an order follow- up inspection on it. Victor McKenzie Drilling Company was issued a chiefs order on this well to plug it on February 20 2018. There has been no change in the status of the well. The well is still sitting idle and not capable of production. I called the company and left them a message to call me back while I was on location. I explained on the message that I was calling about the chiefs order on this well.

Today the rig crew had a solvent truck come out on location to try and clean the paraffin out of the tubing. They are still working on getting the tubing clean so they can have someone come out to shoot the tubing off because it is hung in the well.

Today the rig crew ran the 8 inch pre-cast plug down to 130 feet then used a cement truck to finish plugging the well. The surface plug was filled from 130 feet the surface with 9 sack grout. The well is now plugged.
2101505531 2018-06-29 Plug / Plug Back No Violations I was on location today to meet the rig crew when they brought the rig and the equipment in. Benny Altier has been hired to plug this well. Today they got the rig moved in and the plugging equipment. The landowner is very happy to get this well plugged.
