0578876656 2017-12-13 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-3117. Inspector John Fleming was present during the inspection. A review of RBDMS indicated production reporting was current. Access to the well was through a residential lot. The well head was enclosed by a locked board fence enclosure. Correct identification was present. The 8.625 well head was above grade and open to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. All production equipment was within a locked chain-link fence enclosure. The location was shared with #2-2260. Correct identification was present. A worn earthen dike that was free of fluid was present around the equipment. Equipment within the dike consisted of one (1) vertical separator and one (1) 100 bbl production tank. The separator remained in good repair and contained a current 7 day production chart. The production tank remained in good repair. Valves were closed, locked and plugged. A lightening arrestor was present. While on location Inspector Fleming called and spoke with lease operator George Strawn. Mr. Strawn reported the two wells made minimal amounts of fluid. At the time of inspection, Mr. Strawn reported the tank held 3 feet and 1 inch of fluid. The condition of the dike did not present a clear and present hazard. No improvements to the dike would be necessary at this time. No violations were observed during this inspection.
0604993404 2010-12-01 UrbanPreliminary Restoration No Violations I received a phone call from April Bott(Bott Law Group LLC 614-832-2627) about coming into the drilling location from another way. Ms. Bott was concerned about the traffic on Cranberry Ct. Everflow contacted me about changing the access for the frac only to come off of Cranberry Court and not Indianola road. I approved of this move due to the traffic and school buses that travel this road. This property is owned by Colemans. Everflow had received approval from them about the access change. Ms. Bott wanted the frac to stop and the frac tanks moved away from her brothers property which is the last house on Cranberry Ct, south side. I told her to contact Jay Cheslock about the matter. I requested from Everflow a letter stating that Everflow request a change in access from Indianola road to Cranberry Ct. and a letter from the Boardman Township that shows they approve of this change. I revised the pre-site on this well to show the temporary change in the access to this well. At 9:00AM I emailed Ms. Bott about the revised pre-site showing the temporary access. At 1:57PM I emailed Ms. Bott 2 letters, one from Everflow requesting the change and one from Boardman Township approving of the change. The Boardman statement was With regards to the township, we agree with this request and feel it is in the best interests of our residents due to the inclement weather conditions and for traffic and safety considerations due to the school bus traffic. Ms. Continued to ask for more information. She wanted the cover letter or fax cover letter to these 2 letters. I told her in an email that there were no cover letters. Ms. Bott wanted to know how I received the letters. I answered by saying the letters were hand delivered to me at the location. I have received no further correspondence. This complaint is resolved.

1258093834 2010-11-08 Drill / Deepen / Reopen No Violations I received a call from Mrs. Bonnie Dulton of 761 Indianola road. Mrs Dulton wanted to know why she wasnt told about the rig coming in. She was not in the unit. I made a copy of the unit boundary to show her. She still didnt like it but she said she couldnt do anything about it and thanked me for coming out. This complaint is resolved.

1569722738 2010-11-16 Drill / Deepen / Reopen No Violations I received a call from Ms. Renee Centrello of 1230 Thalia Ave, Boardman about her water well. Her residence is some 1/4 mile north of the drilling location. Her concern was that the well may have done something to her water well and would like the water well checked. She stated that back in 1988 Everflow drilled a well and messed up her water well for sometime. I told her I would call Kyle Tharp, the Geologist for the state. I called Kyle about this well. He would call her to set up a time next week to test her water well. I called Ms. Centrello back to tell her about next week. She had called Everflow about this well. Everflow told her that this well is almost drilled. She did not believe them. I told her the drilling depth was 5361. She was amazed that she did not see the rig until yesterday. I told her that we had set surface on this well last week. Kyle will test well next week. The complaint number is 2912. This case continues.
