0036175691 2010-05-19 Production Wells Violations Noted On site with Jim Smith. Jim dug up the 8 5/8 pipe, leak appeared to be coming from around the ring at the 4.5 inch pipe and annulus. The annulus is tied into the flow line, the flow line valve was closed. Took union after the valve apart, put an ODNR DMRM needle gauge on the line, annulus pressure was 10 psi. Operator opened valves, went to tank battery, opened valve to the tank battery, flow line at the tank battery had a leak in it. Per the operator, there is a leak in the 4.5 inch pipe. 4.5 inch pipe needs repaired or well must be plugged. Tank battery, all line leaks need repaired. There is a cement under ground tank, south east corner of the battery, the tank is no longer in use. The tank is partially submerged. Tank needs removed. There is a depression on the east side of the dike, that is allowing water out of the dike. Tank battery dike needs to be sufficient to prevent the escape of brine and oil. Two wells are produced into one tank battery, each well needs its own identification sign. Tank battery needs to be kept free of water and vegetation.
0712717443 2010-05-18 Production Wells No Violations On site in regards to an odor complaint in the area. Well appears to be leaking around the annuals. Gas was bubbling up around the well head. There is a broken bail of straw around the well head. Removed bail of hay, there is a foot of water covering annulus. Called company was transferred to Wally, last name not given. Wally took my information was contacted by Carl, Carl will have personnel on site. Personnel will contact me when they are on site. Suggested to Carl that the well may have a casing issue. Well does have a casing issue, operator has plumbed well into the flow line. Casing needs repaired.
1283971813 2010-09-07 Plug / Plug Back No Violations I witnessed the plugging of the Big Lime, Berea, surface and the cutting off of the 8 5/8 casing. I also witnessed the vacuuming out and filling of the pit. There were no violations at this time.
1284389759 2010-09-03 Plug / Plug Back No Violations On site for plug job. Operator tripped tubing out, removed rabbit, tallied tubing, tripped back into hole, broke circulation and ran cement. Cement weight actual 15.6lb. Operator tripped tubing out of hole, shut down.
1289323892 2010-11-09 Final Restoration No Violations Site has been seeded and graded. Vegetation is established. Flow line was left under ground. Line was flushed before it Was capped. Waiver needs submitted for pipe line. Once that is received reclamation can be approved.
