1442335465 2015-09-15 Plug / Plug Back No Violations Bishop Well Service ran 2-3/8 inch diameter tubing in to 7320 ft. and set the bottom plug from 7320 to 6970 ft. on 9/14/2015- Formation Cement Service set the bottom plug with 30 sacks of class A at 15.6 lb. and a yield of 1.18 then the well was shut in. Bishop Wall tagged the top of the plug at 6900 ft. but could not rip the 4-1/2 inch diameter casing. They called and I advised that the 4-1/2 inch diameter casing be perforated and a plug squeezed to cover the Clinton formation. I arrived today and Timco Wireline shot the casing at 4916 ft. Bishop Well ran 2-3/8 inch diameter tubing to 4936 ft. and Formation Cementing established circulation and set a 200 ft. plug with 54 sacks of class A cement at 15.6 lb. and a yield of 1.18 - Bishop Well pulled 35 joints of tubing out and Formation reverse circulated any cement out of the tubing, then the well was packed off. Formation squeezed 7 bbl. Of cement into the perfs at 4916 ft. The tubing was pulled and the well shut in for the night.
1442417889 2015-09-16 Plug / Plug Back No Violations Bishop Well Service ran in this morning and tagged the top of the cement plug which was set at 4916 ft.on 9/15/2015 and found the cement top at 4700 ft. The plug after being set was squeezed back into the perfs and 7 barrels of good cement made it to the annular side of the perforated 4-1/2 inch diameter casing. That would be approximately 30 sacks of cement in the annulus. After tagging the plug Bishops Well Service was able to rip the 4-1/2 casing at 4350 ft. and pulled the casing up to 3513 ft. and set a 300 ft. combined plug to 3213 ft. cemented with 75 sacks of class A cement at 14.7 and a yield of 1.36 Next pull casing up to 1516 ft. and set a 200 ft. to 1316 ft. Last plug set at 200 ft. to 0 ft. and circulated cement to surface. All the plugs set today were set using the 4-1/2 inch casing.
1462452668 2016-05-05 Final Restoration No Violations On the day of my inspection I found that the final restoration has been completed. The site passed final restoration according to ORC 1509.072 (B)
