1123329685 2007-07-09 Preliminary Restoration No Violations On site regarding a complaint received by Wayne Schalk DMRM supervisor. Complaint was that there is a gas leak at the well. Inspection findings, there is a small leak on the well head. I was able to hear the leak and feel it around the first hammer union below the rods. I called Dave Hill and left a message on his voice mail that there is a leak at the well and the fitting needs tightened. Site has been seeded and mulched. Vegetation is beginning to become established. Once vegetation is more advanced silt fence can be removed. Tank battery is in good repair. Two 110 BBL tanks and mechanical separator on site. Charcoal filter and lighting arrestors are in place. Lease road is stable and in good repair.
1159459923 2006-09-28 Drill / Deepen / Reopen No Violations Drilling was active. Operator had constructed a ground pit for cement overflow. Pit water level was at ground level. I instructed drill operator to lower the water level in the pit. Operator is having tanks brought onto site. Ground pit will be pumped dry. No identification sign was posted at site. Permit was on site.

A complaint was received by phone. Called operator regarding complaint. Equipment was on site cleaning roads at the time of inspection. Operator will be bring a load of stone, for the lease road.
1179939087 2007-05-23 Preliminary Restoration No Violations Tank battery is installed. Two 110BBL tanks on site, charcoal filter in place, lightning arrestors in place. Well head is completed and pump jack installed. Frack tanks and water tanks on site. Lease road not restored. Well head area is not restored, no grading and seeded has taken place. . Silt fence is up.
1389313390 2017-02-28 UrbanProduction Wells No Violations During this inspection, the well was equipped with an electric motor on the pumpjack and the valves on the wellhead were open. The electric has been shut off and disconnected from the pumpjack motor. There was a domestic line off of the 4.5 inch casing with a meter, drip bottle and two regulators with open valves. The stuffing box was covered in old oil and needs to be addressed and cleaned up. The last reported production for this well was in 2015. The tank battery consisted of one 100 BBL production tank, one 100 BBL brine tank, and a mechanical separator. Valves on the separator were open and the sales line within the tank battery had a chart that showed production. Tank valves were equipped with flat plugs. The vent stack on the production tank was plumbed into a 55 gallon charcoal filter then plumbed up and topped with a spark arrestor. The correct lids and hatches were installed on the tanks. The tank battery was diked and fenced with a locked man door. Proper, legible signage was located at the tank battery and the lease access was stable. I contacted John Suhar from David R. Hill about cleaning up the stuffing box.

1738150329 2006-09-26 Drill / Deepen / Reopen No Violations Status check inspection to check on the steel drilling pits. The mud tanks, shale shaker, and cuttings tank were leased from Union Drilling. I spoke with Kilbargers tool pusher and asked him to place a liner under the cuttings tank.
1893017353 2013-07-15 UrbanProduction Wells No Violations My inspection found the well to be equipped with a pump jack. The annulus valves is open. The well and tank battery are in good repair and are surrounded by a locked chain link fence. The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The production tank is vented through a 55 gallon charcoal filter and up into a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.
2033380007 2007-06-11 Preliminary Restoration No Violations Site has been ruff graded, silt fence is up. Tank battery is installed, and is in the process of being painted. Final grading and seeding has not been completed.
