At the time of this inspection I observed the following elements: A wellhead with pump jack system, a mechanical separator, and a tank battery system. The battery and separator are in a chain-link enclosure. Access to the wellhead is via the shared, asphalt-paved driveway. Fittings and connections at the well and battery were secure and free from evidence of problems or leakage. The battery enclosure gate was unlocked. The load-line valves are not secured with locks. There is an unused steel storage tank stored on-site which must be removed. The owner must do the following by 04/04/2016: remove the unused tank, lock the enclosure gates and load line valves. 0559694858 2017-09-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: the load-lines were not secured with locks, an unused and discarded tank, located outside the battery, was left at the location. In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.
1312712864 2017-12-12 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: The load lines were not locked and secure, and there had been a discarded tank left on site. At the time of this inspection, I observed the following: the noted deficiencies remained. In addition, the unit signage had not been updated since the change of owner that had occurred in 2016 and there was not fencing or enclosure at the wellhead. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
1351522052 2012-10-26 UrbanProduction Wells No Violations The unit consists of a 1-tank battery (TB) with mechanical separator enclosed in a 6-foot chain link fence. The spacing is in accordance with the rule, and the valves and mechanicals are in good repair. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a pumpjack and is not enclosed. The gates on the enclosures are not locked. The tank battery dike is adequate; however, the brine collection tank is outside the diked area. The access hatch is missing from the brine tank. I called Tim Reilly of Great Plains and advised him about the need for locks and dike containment for the brine tank. I indicated that the items should be addressed by years end.
1802087394 2017-07-11 UrbanProduction Wells No Violations I inspected this producing well location as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had noted the following: the tank battery enclosure and load-line valves were unlocked and not secure as required. An unused field tank is sitting discarded near the well. These conditions remain at the time of this inspection. Also, I noted that the ID signage reflects the prior owner and remains not-updated since the transfer of ownership in 12/2016.
1837489156 2017-04-19 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves and a discarded tank outside the containment. At the time of this inspection, I observed the following: The load-line valves were not secured with locks. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). The discarded tank was still on the location. I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
