1015912503 2013-06-19 Preliminary Restoration No Violations The producing unit consists of the following elements: A production wellhead and storage tank battery. The production wellhead is fitted with a mechanical plunger lift system and is located 100 feet west of the tank battery. The tank battery system includes 1 100-barrel storage tank, a 25-barrel poly tank, mechanical separator and associated appurtenances. The tank battery is enclosed in a chain link enclosure. The enclosure is locked and the secondary containment is good. Tank and well spacing is in accordance with 1501:09.

1123648855 2019-03-07 UrbanProduction Wells No Violations I conducted an inspection on this well. The well was equipped with a plunger lift assembly and in production, at the time of this inspection. The gate valve on the tubing and the 2 inch flow line valve were open. There was a domestic line plumbed off the 4.5 inch casing with a regulator, drip bottle and domestic meter. The valve on the domestic line was open. The well was fenced. The tank battery was comprised of one 100 BBL tank, one 25 BBL tank and a mechanical separator. Valves on the separator were open and the chart in the separator showed production. The diked tank battery was fenced. Legible identification was present at the well and tank battery and the lease access was stable.
1382376214 2013-10-21 UrbanProduction Wells No Violations The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery. The battery is comprised of 1 140-barrel steel production tanks and 1 25-barrel brine tank, and separator unit. The wellhead and battery are both enclosed in separate chain-link fences. The load-outs are bull-plugged. The ID and secondary containment are good. The lease road entry is gated and locked and requires an ID placard. No other problems noted.
1689834775 2017-11-29 Production Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located on an otherwise undeloped parcel, with newer residential development in progress in the surrounding areas. At the time of this inspection, I found the following: a wellhead plunger-lift unit and associated battery in separate chian-link enclosures. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The shared-use access road was in good condition. The well was in active production. The chart recorder was active. Production reports and insurance were current. I noted that the heater tube at the bottom of the production tank had a 4-inch rubber Fernco fitting affixed to fix leaking fluid. I noted a small amount of lekage of water/brine from the fitting. I contacted Matt Postelwaite with Alliance and advised about for mitigation on the heater tube leak. He indicated that he would tend to it ASAP. I met with land owner Mark Murch who reported no problems or issues with the well or with Alliance Petroleum.
