0414921149 2014-08-13 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 08/17/2007. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system and aboveground storage tank (AST) battery in separate board-on-board enclosures. The wellhead and surrounding area are free of signs of leaks or other impacts. The wellhead and battery are located in a sparsely developed portion of Willoughby Twp. The area surround the wellhead and battery is heavily wooded. Access to the wellhead and battery are via a dedicated gravel access road which travels north from Hayes Dr., through a gate, and terminates at the wellhead and battery in 500 . The battery is comprised of 1 steel 210-barrel production tank, 1 100-barrel steel brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. Surface drainage from the area is to the north, towards the Chagrin River, 500 feet southeast. I did not observe any gully formation or other surface drainage erosional deficits. The ID placards at the wellhead and battery are in good condition.
0629457035 2018-02-16 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves, missing process safety equipment (low-pressure relief valve, spark arrestor, carbon filter), as well as missing ID signage. At the time of this inspection, I observed the following: The previously noted missing process safety equipment remained missing. The load-line valves were not secured with locks. The wellhead and separator enclosures were locked and secure. In addition, the ID signage had not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted and met with the owner regarding these deficiencies. The owner has pledged to remedy the problems.

I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
0752526454 2016-02-29 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following elements: one wellhead with pump jack system, one mechanical separator, and a tank battery comprised of one 210-barrel steel tank, one 100-barrel steel tank, and mechanical separator, with associated fittings and tubular connections. Secondary containment was intact. Access to the well is afforded by a short dedicated, unpaved access road which enters from Hayes Drive. At the time of this inspection, the road was in good condition. Tank lids were closed and sealed. Secondary containment was intact. Load lines were plugged but not secured with locks. The well ID was partially missing and not readable. There was no spark arrestor, low-pressure relief valve, or carbon filter. The owner must remedy the following by 02/30/2016: Secure the enclosure gates and load-line valves with locks, fit the tank battery system with carbon filter, low-pressure relief valve, and spark arrestor, replace the missing well ID.
