At the time of this inspection, access to the well was restricted by a locked gate to the property. I attempted to contact David Snyder. There was no answer to several attempts to contact via telephone.
0289439141 2018-01-23 UrbanProduction Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-560 for failure to maintain liability insurance. At the time of this inspection, the gates to the property were locked and secure. The property was being listed for sale and I contacted the realtor to gain access. I also contacted Dominion Energy to gain access to the metering vault located outside the property entrance. A response to these requests was pending at the time of this report filing.
0377990144 2018-01-19 UrbanProduction Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-560 for failure to maintain liability insurance. At the time of this inspection, I met with Seth Young (Century 21) to gain access to the property, and Dominion Energy crews to gain access to the metering vault. I inspected the well and found the flow-line connections and fittings secure. Effluent valves were open and capable of supplying gas back to the metering vault, however I found the flow-line closed at the metering vault. According to production reports, the well produced and sold gas into the Dominion Energy network in 2016.
1296233825 2017-09-28 UrbanProduction Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-560 for failure to maintain liability insurance. At the time of this inspection, I observed the following: a wellhead with flow regulator and flowline connections in a circular, subsurface vault, located along the north perimeter of the residential property. The flow-line connections were closed. The well was not being produced.
1418935452 2014-12-18 Exempt Well Pre Transfer No Violations I inspected this well per request of ODNR, Division of Oil and Gas, Permitting Division. The well is located in the back (north side) of a house on Edgewater Drive. Visible elements at the wellhead consist of 5-inch casing riser with hammer union to 3-inch production casing to reducer swage and 1 7/8-inch supply line. The well is located below grade in the 60-inch diameter concrete vault structure with a hinged metal grating cover. There are two in-line gate valves which are both in the open position. The supply line leaves the well via a penetration through the vault side wall. According to Ed Grant, the property owner, the house is currently unoccupied. The house had formerly been rented to Dave Snyder, but that arrangement has ended.
1527803038 2018-05-31 Exempt Well Pre Transfer No Violations I inspected this well location as part of a requested transfer of ownership and change of status to exempted domestic. At the time of my inspection, I found the following: The well was located in the back (north side) of a house on Edgewater Drive. Visible elements at the wellhead consisted of 5-inch casing riser with hammer union to 3-inch production casing to reducer swage and 1 7/8-inch supply line. The well was located below grade in a 60-inch diameter concrete vault structure with a hinged metal grating cover. There were two in-line gate valves which were both in the open position. The supply line leaves the well via a penetration through the vault side wall. The supply connections at the exterior wall of the house were closed and not active. The house was vacant with no current occupant at the time of this inspection. There was an active commercial sales line connection located near the street, in a small, free-standing metering shed, with a commercial-grade metering system with extant connection to Dominion Energy distribution system. The sales line connection was closed. The measured distance from the well to the west, adjacent neighboring residential dwelling was 146 feet. The measured distance from the well to the east, adjacent residential dwelling was 196 feet.
