2658031 Primary Facility 2017-11-01 Routine/Complete Inspection No Violations Noted I conducted a routine inspection at Energy Corporation of America (ECA) COP Tract 324 A Pad covered under ESX11-033-0038 on November 1, 2017. No one was onsite during my inspection.

There are currently two producing Marcellus wells located on this partially restored well pad (033-27206 and 033-27221). Well cellars are full of water. Field screening for conductivity in each well cellar were 202 uS/cm and 186 uS/cm respectively. Ponded water was observed throughout the partially restored well pad (see photos). Conductivity field screen readings were 200 uS/cm, 208 uS/cm, and 215 uS/cm.

Produced fluids tanks are located in an earthen dike (see photos). There is approximately 1.5 ft. of water present within the dike. Specific conductance within the dike was 176 uS/cm. I recommend removing some of the water within the earthen dike.

The partially restored well pad and surrounding areas was seeded earlier in the summer of 2017. Currently the site does not meet 70%, uniform, perennial, vegetative cover.

2764469 Primary Facility 2018-08-14 Routine/Complete Inspection No Violations Noted Department staff Sue Herzing, Sara Metz, and Mike Jurbala conducted a routine site inspection at Greylock Production LLC COP Tract 324 A Pad covered under ESX11-033-0038(01) on August 14, 2018.

The well pad was partially reduced/restored during early Summer 2017. The site meets 70% uniform, perennial, vegetative cover. I recommend submitting a Notice of Termination for the ESCGP.

There are currently two producing Marcellus wells located on this partially restored well pad (033-27206 and 033-27221). Well cellars are full of water. Bubbling was observed in well cellar 033-27206. I will be notifying Department Oil & Gas inspector Ulyana Trynovich regarding the bubbling. The Department recommends that the operator investigate the well cellar bubbling. Field screening for conductivity in each well cellar were 156 uS/cm and 248 uS/cm respectively.

Produced fluids tanks are located in an earthen dike (see photos). There is approximately 1 ft. of water present within the dike. Specific conductance within the
