There is no motor on the pumping unit. The well is currently shut in. There is identification information posted at the well head and the 100 bbl production tank, but no API number is listed. According to RBDMS records, this well has never had production records submitted. This well needs to be plugged. The operator will have 180 days upon receipt of this notice to plug and abandon the well. |

It was decided that the best option in this case will be to haul the soil away to an EPA approved landfill for disposal. A new 100 bbl production tank was brought onto location this morning and I approved a new location for the tank which met division spacing requirements. Plumbing of new flow lines will happen over the coming week and a containment dike will be constructed around it to meet SPCC guidelines. The affected area of the old tank pad has been seeded and mulched. There is still sheen showing in Fifteenmile Creek and the small tributary, but is significantly less than it was on Monday and Tuesday. 1530019010 | 2015-06-28 | Production Wells | Violations Noted | Received a call from Mr. Bobby Kroll at 7:42 pm reporting an oil spill on this lease which had impacted Fifteenmile Creek and a small tributary. Upon arrival at 9:15 pm, it was observed that an estimated 3-5 bbls of crude oil had leaked out of the bottom of the 100 bbl production tank, seeped through the soil down to bedrock, and migrated into a nearby tributary and into Fifteenmile Creek. Cleanup efforts had already commenced upon arrival and a series of soft booms, absorbent pads, and straw bales were installed in the streams in select locations up to .25 miles downstream. An oil gathering truck was brought onto location to pump the production tank to empty and prevent further release. In addition, a slight brine kill approximately 6 x 20 was observed leading downhill from a small knockout tank located uphill from the production tank. OEPA DERR was notified of the spill and Ms. Rikki Knerr arrived on location at 11:20 pm to assess the impact on streams. The operator must meet the following conditions within 10 days upon receipt of this notice: 1.) All contaminated soil shall be excavated and either bio-remediated on location with lime/fertilizer for re-use or disposed of at an EPA approved landfill. 2.) All free oil product shall be removed from Fifteenmile Creek and the small tributary. 3.) The production tank shall be relocated to a location at least 50 feet from any stream as per ORC 1509.021(L). 4.) An SPCC compliant containment dike shall be erected around the new tank location. 5.) The brine knockout tank used for the lease shall flow all produced fluids into either the production tank or a separate brine storage tank. |

The temporary sump was evacuated of any residual crude oil, which was very minimal, and the entire affected area was treated with lime to adjust pH and encourage bioremediation of any residual hydrocarbons in the soil. All clean soil that was preserved from the excavation was used to fill the sump back in and put the affected area back to grade. Lime was applied to all of the preserved soil as well to help encourage the regrowth of healthy vegetation. Hard and soft booms are still present in Fifteenmile Creek and are still recovering a considerable amount of free product and sheen. I was accompanied today by SW Region Emergency Operations Manager Trisha Cartmell. |

I informed Mr. Hamilton that the notice will be closed once vegetation is established on the new tank pad area and record of disposal for all contaminated soil is provided to the division. |
