0048319395 | 2011-06-01 | Production Wells | No Violations | FOLLOW UP TO NOV # 1306967521 Upon receiving complaint from landowner of idle well on property, investigated site and found well to be idle last production reported on well was back in 1999 and well is equipped with pumping unit and tubing and rods and 1-100 bbl tank in need of dike if well is placed back into production.
Plug or Produce well or file for temporary inactive status. Failure to comply may result in bond forfeiture. |
0152566829 | 2019-04-29 | Production Wells | No Violations | Purpose of todays inspection was PRE-SITE for potential injection well operations.
Well is currently equipped with pumping unit and tubing and rods and one 1-100 bbl. tank and separator. Once converted injection line will need to be double encased to ensure that in event of line leak nothing enters into waterways. |
0260972299 | 2014-09-16 | Administrative Inspection | No Violations | I stopped at Green Gass shop and spoke to Scott Davis field superintendent. Mr. Davis stated the well was transferred to the land owner. I told Mr. Davis to file a form 7 with the division. |
0614979064 | 2017-02-13 | Production Wells | No Violations | The purpose of my inspection today is a status check on a compliance notice numbered 1306967521 that was issued to Green Gas Company. The compliance notice stated the well is in violation of 1509.12(B), 1509.062, 1501:9-9-05(A) 10, and Failure to properly bury pipeline 9-10-05. During my inspection the well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 1.5 inch diameter tubing, rods, and a pumping unit with a natural gas motor. The tubing valve is closed and the production valve is closed. The production equipment consists of one 100 barrel tank, one drip, one vertical separator, and one chart meter. There has been no action taken to gain compliance. The well is idle. Pictures were taken to assist this inspection. |
0967450044 | 2019-03-20 | Production Wells | No Violations | The purpose of my inspection was to follow up on a previously issued compliance notice. Production equipment included a 100-barrel steel production fluid storage tank, vertical separator, and 2-inch diameter meter run with chart recorder, there was no chart in the chart recorder at the time of my inspection there was no earthen containment around production equipment. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 1.5-inch diameter tubing and rods with an Emsco pumping unit. At the time of my inspection all valves were closed, the bridal cable was no longer attached to the rods and there was a large tree growing inside of the pumping unit. Identification was on the production equipment with Buckeye Oil Producing on the Identification, and the production has been reported as 0 since 2002. |
1036866346 | 2017-05-09 | Production Wells | No Violations | The purpose of my inspection was a status check on compliance notice 1306967521. The compliance notice issued to Green Gas Company in 2011 stated that the well was in violation of 1509.12(B) Well shall be plugged, 1509.062 (A) Nonproduction, 1501:9-9-05 (A) (10) Failure to legibly identify well, and 1501:9-10-05 Failure to properly bury pipeline. During my inspection the well was equipped with 4.5 inch diameter production casing with a tubing head, 1.5 inch diameter tubing, rods, and a pumping unit with a natural gas motor. All valves were in the closed position. The production equipment consists of one 100 barrel tank, one drip, one vertical separator, and one chart meter. Since the year 2002 to the year 2014 Green Gas has reported all zeros for production. The well was not legibly identified in a conspicuous place on or near the wellhead or the storage tank. Multiple steel 2 inch diameter production lines are laying on the surface near the production equipment and lease road. No action has been taken to gain compliance and all of the original violations from the compliance notice 1306967521 still exist. Pictures were taken to assist this inspection. |
1106101837 | 2015-09-29 | Administrative Inspection | No Violations | At 2:29 pm I received a call from Scott Davis in response to voice mail I left for him earlier in the day. Mr. Davis and I discussed the violation against the well. Mr. Davis stated that the well was given to the landowner. I again told Mr. Davis that Green Gas was responsible for filing a form 7. Mr. Davis stated that he would see to it that it was done. |

1558362688 | 2019-05-20 | Production Wells | No Violations | On site follow up inspection to violation # 1306967521 issued on 06/01/2011 by Inspector Dave Ball. It was observed that a service rig owned by Perkins Oil Field Service out of West Virginia is on location and rigged up on the hole. On site was Company representative Rick Zickefoose. The rig has pulled 5,800 of 1.900 production tubing out of the well bore, the well was set up to be produced by the means of a pumping unit with 5/8 diameter pumping rods also recovered out of the well. The tubing had approximately 200 of debris fill up reported around the tubing at the bottom of the hole. It was reported that the 4.5diameter production casing was loaded with brine water while recovering the tubing and the rig was able to get to bottom with a 3.125diameter sand pump on the sand line. The rig then attempted to swab down the 4.5 and was not able to get past 2,800. They are currently sand pumping what appears to be grey shale from the inside of the 4.5 diameter production casing at 2,837. Mr. Zickefoose indicated the surface casing well head section was standing open with no valves present when they first uncovered the well head assembly. They have installed valves on the surface casing. No surface casing pressure has been reported to date. The rig will continue to sand pump on the well and Mr. Zickefoose will update me with the progressing status moving forward. |
1558541775 | 2019-05-22 | Production Wells | No Violations | Status check inspection found a service rig on the hole owned by Perkins Oil Filed Service out of WV. The sig is sand pumping shale cuttings from inside the 4.5 production casing at a depth of 3,453. |
1559226236 | 2019-05-30 | Production Wells | No Violations | Status check inspection found a service rig owned by Perkins Oil Field Service out of WV rigged up on the hole sand pumping the inside of the 4.5 diameter production casing at 5,500. Total depth to get past the perforations will be 5,900 then an injection test will be initiated. |
1915944375 | 2013-10-10 | Administrative Inspection | No Violations | I spoke to Scott Davis of Green Gas on 10-10-2013. Mr. Davis stated that they no longer own the well. He stated that the well was released to the land owner. Mr. Davis also stated that his brother was preparing the form 7 to file with the state. |
