1499700152 | 2017-07-10 | Production Wells | No Violations | Status check inspection found the well equipped with 5.187 diameter production casing and 2 diameter production tubing with the well set up to be produced by the means of a pumping unit with only the tubing head sales line connected. On location near the well head is the production facility consisting of a steel construction 100 barrel capacity storage tank. No dike is present around the tank and the well and tank sits in close proximity to Sunfish Creek. No commercial production has been reported since 2014. |
1525099269 | 2018-04-30 | Production Wells | Violations Noted | Status check inspection found the well equipped with 5.187 diameter production casing and 2.375 diameter production tubing. There is both casing and tubing well heads present however the only flow line present is attached to the tubing well head section. The well is set up to be produced by the means of a pumping unit. There is a larger area of stressed vegetation that is dark color in front of the well head. On location near the well head is the production facility consisting of a single 100 barrel steel construction tank. Situated atop the 100 barrel production tank is a blue drum that has been converted into a gravity fluid separator with a 2 primary line leading from the drum into the 100 barrel production tank and a 1 plastic discharge line that is connected to the blue drum in a way to allow a unimpeded flow to discharge a measured distance of 62 from the production tank into a depressed area on the earths surface where flowing water is present. At a location of 39.76409 x 81.21253 the water in the depressed area was titrate tested for chlorides with both High and Low level test strips with the chloride levels exceeding the low level titrate test strips at 637 parts per million and reading 1,061 parts per million on high range strips. Stressed vegetation was present inside the depressed area. Also noted is there is multiple wells feeding into the same common tank and as described in 1501:9-9-05 (10) each well -head shall be identified as to owner and permit number and lease name. My direct supervisor Brent Bear was notified and requested I contact the producer and inform them to stop the discharge into the waters of the state. I then contacted a Derick Ludwig and informed him to cease discharge of well bore products into the waters of the state and address the pollution and contamination in front of the well head. As a follow up to the conversation the violation is being issued with instructions as follows - immediately stop the discharge into the waters of |
1531761998 | 2018-07-16 | Production Wells | No Violations | Compliance notice # 1525099269 follow up inspection found the area in front of the well head identified in the previous inspection with pollution and contamination has been addressed and vegetation is established. The discharge line from the production tank has been removed from the waters of the State. The previous violation is considered resolved and has been closed out on this day. |
2061558197 | 2018-05-17 | Production Wells | No Violations | Compliance notice # 1525099269 follow up inspection found the area in front of the well head identified in the previous inspection has the contaminated material removed. However no identification has been posted at the well head. Also at the time of my last inspecting the casing below the well head a ground level was not visible and it appears as though the company has removed the hay they previously placed around the well head to reveal the well does have a 6.625 casing string present that has been cut off at ground level. The annular space between the 5.187 outside diameter and inside of 6.625 casing is standing open and has crude oil present. Also the 1 diameter line leading from the knock out separator to the ground surface is still present and has not been removed and is still placed in a manner to allow well bore fluid to be dispersed into the waters of the state. |
2141132159 | 2018-05-17 | Production Wells | No Violations | On this day, I accompanied ODNR Inspector William Stolarik on a Compliance Notice follow-up inspection. Please refer to his comments for a detailed report. |
