On 5/29/2013, a Baker packer was installed on the 2.375 injection tubing and Petroset Cementing Services circulated a packer fluid containing an oxygen scavenger, rust inhibitor and biocide. After successfully completing a Mechanical Integrity test the well was shut in awaiting further conversion completion. My inspection ( 9/25/2017) indicated that the ONeil # 3-D well was incomplete and incapable of injection for Enhance Recovery. The wellhead consisted of 4.5 casing with 2.375 injection tubing inside. The injection tubing was capped with a Demco gate valve ( in the closed position ) and a gauge that did not indicate any pressure. The annular ( 4.5 X 2.375 ) ports had two (2) closed wing valves. The wellhead was surrounded with a chain link fence and overgrown vegetation. I did not identify any leaks at the wellhead. The I D signage was posted and legible. The wellhead was not connected to any injection lines. The ONeil # 3-D did not have any injection pumping or storage tank facilities. The OAC 1501:9-5-05-(I)(1) states If drilling or conversion operations have started but are not complete within the twelve month period, operations shall continue with due diligence or the permit shall expire. The permit issued on 6/13/2012 for conversion has expired. The Well Owner/ Operator of Permit 34-153-2-2928, within 14 days, shall submit to the Division of Oil and Gas Resource Management at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio, 44685 a plan detailing the future utilization of the well along with a time frame of said utilization plan.
