1984003 2011-06-27 No Violations Noted Drilling was ongoing at the time of the inspection. Drill rig crew indicated that they had finished setting the conductor pipe. Drill pit located onsite. It had more than the required two feet of lined freeboard. Noted that the erosion controls had been implemented as outlined in the E&S plan. The operator should perform routine erosion control inspections and maintain the controls as necessary to insure their continued effectiveness.
1994772 2011-07-28 No Violations Noted Drilling appeared to have been completed. Construction was underway for the well stimulation impoundment. All E&S controls had functioned properly. No accelerated erosion or sedimentation was observed in relation to the well-site. Drill pit onsite contained fluids and drill cuttings. Pit had more than the required two feet of lined freeboard.
2001553 2011-09-08 No Violations Noted Drilling appeared to have been completed. Operator was in the process of moving a service rig and other equipment onsite. Due to recent weather conditions this has resulted in the transport of large chunks of mud onto State Route 1034. Contacted operator representative Doug Miller regarding this matter and he stated the contractor has already been notified of this matter and a plow and/or broom truck was to be onsite addressing the mud on the road issue. Mr. Miller requested that if WQS Higgins did not observe the truck onsite in the near future to call him back. Upon leaving the site the plow truck in question was en-route to address the issue. The operator should evaluate the access road erosion controls (i.e. rock due to lack of grade associated with the road) after this period of heavier traffic is completed.
2018791 2011-11-14 No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Both well stimulation and drill pits located onsite. Both pits contained fluids and had more than the required two feet of lined freeboard. It was noted that there were several small holes observed on the sidewall of the well stimulation pit. The fluid level in this pit should be kept at least two feet from the lowest hole. All E&S controls appeared to be functioning properly and there was no evidence of accelerated erosion or sedimentation.
2063956 2012-05-01 No Violations Noted Inspection performed as a result of an operator request for an extension to the 9-month restoration timeframe. Drilling had been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Drill pit and frac impoundment located onsite. Both contained fluids and had more than the required two feet of lined freeboard. All erosion controls had been properly maintained and there was no accelerated erosion or sedimentation observed in relation to the access road or well-site. The operator should continue to monitor and maintain the erosion controls until well development activities are completed.
