The affected area of the leak was approximately the size of a sheet of paper. It has not been cleaned up but is not leaking any longer. The annulus pressure is 0 PSI and I need to witness a monthly annulus test to verify that there is not a problem.

The area of the brine spill has been graded but the contaminated soil has not been removed as requested. Mike Demantio said that the contamination of the soil was very slight and he feels it will grow grass. I advised if it did not green up very quickly it would need to be removed. J Hawk water service pumped up the injection annulus and during the pressure test the mini test failed. The 4 1/2 casing had 3 pin hole leaks which caused the test to fail. Mike Demantio said that after the service rig was done on the Stephenson that it would come here next.

1343409103 2012-07-24 Salt Water Injection Well No Violations I am on site today to conduct a mini Mechanical Integrity Test on the annular space between the 4 1/2 inch casing and the 2 3/8 tubing. The annulus was pumped up to 820 psi. The 1st test failed. On the 2nd test, the annulus was pumped up to 820 psi and the pressure reduced to 810 psi where it remained until the test was completed. This would be less than the allowed .05% decline and is a positive test. The area, previously seeded, below the pump house has not produced any vegetation and should be excavated soon, re-seeded and mulched. Mike Demantio said that the vault was being constructed and will be installed next week. I advised Mike that I wanted to be on site for the construction and installation of the vault.

I later met with Mike Demantio and scheduled the clean up and a mini test on the annulus for Monday, 6/25/2012.
