Fuel Co. at 8:00 am stating he would be cleaning up brine contaminated soil around this well head today. I met with Mr. Holden on location at 8:15 am to discuss what needed to be done. I instructed Mr. Holden to dig down 1.5-2 around the well head and work his way away from the well head following the vegetation kill, shallowing the dig depth with more distance from the well. Mr. Holden had 2 bags of lime, 1 bag of fertilizer, grass seed, and straw bales on location. I checked back at 1:30 pm and observed that all contaminated soil had been removed and replaced with clean soil, the affected area had been treated with lime and ferlizer, and Mr. Holden was about to sew grass seed and mulch the area. I will continue to monitor the progress of this cleanup. |

The casing could not be welded due to severe corrosion, so a 2x2 concrete pad was poured over the 12 surface casing to seal it off. A 1 vent line was left exposed through the concrete to allow periodic venting of the 12 casing if necessary. This satisfies the requirements of the compliance notice dated 9/14/2015. I was accompanied on this inspection by DOGRM ER Coordinator Nathan Lane. |

The well consists of 12 surface casing, 4.5 production casing on a packer, and 2.875 tubing with a rabbit catcher. There is brine leaking out of the well on the 12-4.5 annulus. Brine is ponded around the well head area and has created a kill zone 8 wide x 15 long leading away from the well head. I contacted Mr. Steve Holdren of North East Fuel Co. and notified him of the situation. I instructed Mr. Holdren to remove all contaminated soil from around the well head, dispose of it properly, lime the area, replace with clean soil, and seed/mulch the area to encourage re-vegetation. Mr. Holdren stated he would clean address the issues and have the location cleaned up before the end of next week. The tank battery for the well consists of a 100 bbl production tank, mechanical separator, meter run, and containment dike. Legible and corret ID is posted at the tank. |

I will call North East Fuel Co. and inquire as to why the well has not yet been repaired. |
