0083766414 2016-08-18 Production Wells No Violations On the date of my inspection I found the access road gate locked and the access road in good condition. Concrete barriers were in place around the wellhead. Proper identification was posted. The equipment on the pad consists of six shared 400 barrel storage tanks contained in steel dike, 2 active wells, 1 vapor recovery unit, 2 upright sand separators, and 2 gas production units. Annular pressure on the 13.375 inch casing was 27.4 pounds per square inch, pressure on the production 5.5 inch production line was 187.5 pounds per square inch.
1350487425 2012-10-17 Production Wells No Violations My inspection found a gated well pad with two producing wells on location. The site included separators and six 400 barrel tanks surrounded by a steel dike. Adjacent to the well pad is a freshwater holding pond. On site was a crew that was painting the tank battery and flow lines. There are no violations at this time.

1654125923 2016-03-08 Production Wells No Violations My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #3H well in production. Concrete barriers were in place around the wellhead. Well identification number was found on the well and on signage at entrance to access road. A freshwater impoundment adjacent to the pad is fenced and secured, with identification signage attached to the fence. Approximately 3 feet of freeboard was observed. The equipment on the pad consists of the following:

6 400 bbl. storage tanks contained in steel dike. 2 active wells. 1 VRU. 2 Upright Sand Separators. 2 GPUs

2072281894 2014-03-05 Preliminary Restoration No Violations I was at the site to conduct a preliminary restoration inspection. During my inspection, I found the wellhead to be equipped with a Christmas tree valve assembly with all required pressure gauges and valves. The wellhead is in good condition, and is protected from vehicles by concrete barriers. The location has a graveled lease road with a gate, and the pad is fenced and graveled. The pad contains vertical separators and other production equipment. Six 400 bbl production tanks are situated within a coated steel SPCC containment. The erosion controls are in place and functioning, and vegetation is well established. Preliminary restoration is passed.
