0231291962 2018-07-05 Production Wells No Violations The purpose of my inspection today was a status check. During my inspection the well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing with a tubing head, 2.375 inch diameter tubing, and a pumping unit. Production valves were in the open position. The production equipment consisted of one 100 barrel tank, one 1550 gallon poly tank, and a vertical separator. Identification was located on the tank and Luburgh Holdings had submitted reports of oil production in the year 2017.

0554905653 2015-04-08 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a 100bbl tank and 30bbl brine tank within an earthen dike. Also a vertical separator and a 2 inch sales meter with a .25 inch orifice and a chart recorder with no chart present. The well was equipped with 8.625 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and a pumping unit powered by electricity with valves open on the tubing and closed on the casing. Identification was present and production records were current.
