0226322298 2014-06-23 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Universal break circulation with fresh water then mix and displace five hundred forty two barrel of Type-1 cement to a depth of 315 foot, I witnessed 20 barrels of cement returns to surface with a weight of 15.6 and a yield of 1.20. My site inspection revealed no erosion and good vegetation, no spills to report and all fluid storage is setting inside containment.

0480989814 2018-09-25 Production Wells No Violations The purpose of my inspection was to document the status of Utica Resource Operatings Dynamite Pad prior to SIMOPS activities. Guerney County Inspector Austin Gutridge was present during the inspection. At the time of inspection well #2-4337 remained in production. Correct identification was present. A total of four (4) production wells were on the pad. Well #2-4337 will be within the fall radius of the anticipated rig contracted to drill the two permitted wells on the West side of the pad. Approved temporary abandonment permit conditions will need to be completed on well #2-4337 prior to the commencement of drilling activities that include well #2-4337 within the fall radius of the derrick.

During my inspection the well was caged and still appeared to be in production. Rig components were being brought on location.
0530952458 2019-01-11 Production Wells No Violations The inspection was conducted as a simultaneous operation inspection on a production well. The well is equipped with a large Texas Style gas plunger lift system with a steel cage built over the well. The production master valves are open, and the annular valves are open measuring pressure. The tubing pressure is 0 psi and the annulus are reading 0 psi.
0675723424 2018-09-27 Production Wells No Violations Accompanied by ODNR Southeast Regional Supervisor Brent Bear, during our inspection rig mats were being place on the pad. The well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of fifteen 500-barrel tanks, four gas production units, two gas compression skids, four vapor recovery towers, horizontal vent scrubber, and one combustor. Kevin Rothenbuhler contacted me and stated that the pea gravel was removed from the cellar, the valves for annular pressure monitoring were tightened, and while utilizing a gas sniffer no leaks were detected. The pea gravel was placed back in the cellars today and no bubbling was present.
0718330116 2018-09-25 Production Wells No Violations Accompanied by ODNR Kenn Frye, during our inspection a berm was being constructed around the pad with the cuttings from the conductors. Conductors had been cemented for 3405924532 and 3405924533. The well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of fifteen 500-barrel tanks, four gas production units, two gas compression skids, four vapor recovery towers, horizontal vent scrubber, and one combustor. Kevin Rothenbuhler was contacted about the cellars bubbling on the 3H and 4H. Mr. Rothenbuhler stated that they would remove the pea gravel from the cellars on all wells and tighten valves and fittings that are plumbed up for annular pressure monitoring to remediate the bubbling.

1055609994 2018-11-08 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-4337. At the time of my inspection the well was temporarily abandoned and caged while Patterson #343 was rigging up to drill two additional wells on the pad. Jersey barriers were placed outside of the cage to prevent a collision with the well head. Pressure gauges to monitor annular, production, and tubing pressure remained in place. The intermediate casing annular pressure gauge indicated 60 psi. The production casing annulus indicated 0 psi. The casing pressure gauge indicated 900 psi. The tubing gauges indicated 200 psi. All valves remained in the closed position. No leaks were observed. The flare stack was positioned in the Southwest corner of the pad and away from the well head and surface equipment. While I was on location I met with company representative Dave Gray to discuss the condition of the well. I will contact Utica Resources VP of Operations Kevin Rothenbuhler to investigate the situation and make DOGRM Inspector Patrick Shreve aware of the situation for monitoring.
1212700983 2014-03-25 Casing String Witnessed No Violations On the day of my inspection I witnessed that Express Energy Services had drilled a 36 inch diameter hole to the depth of 115 feet and set 115 feet of 30 inch conductor pipe. Express Energy could not get conductor pipe plum. They are pulling pipe back out and will ream hole to 42 inches in diameter and rerun pipe and cement tomorrow.

Gauges present read 375 pounds per square inch on the tubing and 555 pounds per square inch on the casing. Production equipment included a sand trap, a GP Unit, a vapor recovery system and a total of 12-500 barrel tanks all within containment and shared with the 1H, 3H and 4H.
1476978276 2016-10-20 Production Wells No Violations At 0830, I received a call from PDC EHS Supervisor Tony Long who stated that Senecaville Fire Department was dispatched to the location last night as flames were coming out of the combustor. Upon arrival I met with PDC Production Superintendent Kevin Rothenbuhler who stated that the incident occurred this morning approximately at 0700 with the wells being shut-in remotely. Mr. Rothenbuhler stated that when he arrived it was determined that the dump valve for the tower of the 2H malfunctioned resulting in the float sticking. The tower then filled with oil, overflowing into the vapor recovery line, filling up the slug catcher and making its way to the combustor where it was ignited. The vapor recovery towers had been converted to separators approximately 2months ago resulting in reducing the working pressure of the vessel from 260 pounds per square inch to 30 pounds per square inch. There were no visible signs of the fire or the product reaching the well pad. Well technicians were on site making repairs to the dump valve and ECM was pulling fluids from the system. Upon repairs to the valve the wells will be brought back online.

1770352598 2018-10-11 Production Wells No Violations During my inspection Fountain Quail Rig 4 was on location preparing to drill two additional wells on pad. The well was still producing during inspection. I spoke with Dave Grey, the company man on location for Utica Resources and Mr. Grey stated that they plan to produce the well until the super triple rig comes on location to drill the curves and laterals on the other two wells 7H and 8H.
