0437596535 2017-10-17 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains four 400-barrel production fluid holding tanks. The holding tanks and gas processing units are within a fenced in area. Production records for this well are current to 2017 within RBDMS.
0674395211 2018-09-26 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 0 pounds per square inch, the 13.375-inch diameter surface casing gauge read 400 pounds per square inch. Production equipment included a Line Heater, Gas Processing Unit, additional horizontal separator, coalescent filter, and scrubber. There were also 3- 400-barrel steel production fluid tanks, and a 400-barrel plastic production water tank inside secondary containment, that were shared with the rest of the producing wells on the pad. Also, on location was a small flare stack that was flaring at the time of my inspection. Identification tags were not present on the wells and I spoke with Ascent Resources, Doug Bennet who informed me that they were still changing out identification tags from the purchase of Hess. Ascent will have all the wells re-identified in 30 days. Production was current in RBDMS.
1043376960 2015-08-18 Preliminary Restoration No Violations On this day completion operations were underway. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
1412978530 2014-10-10 Casing String Witnessed No Violations On the date of inspection I accompanied ODNR Inspector Pat Shreve to witness the cementing of the surface casing string. Refer to Pat Shreves report for further details.
1413008992 2014-10-10 Casing String Witnessed No Violations Had drilling rig DR24 drilled a 321open hole of 28.5. They ran 315of 18.62587.5lb/ft., Buttress thread J-55 casing. There was 100 Of 30conductor drilled in concentric. There was 9 centralizers ran on the string of 18.625casing and a guide shoe. Halliburton ran a Halcem blend of 1050 sacks with a weight of 15.6ppg and a yield of 1.21. There was good circulation with 44 barrel of returns.
1889106207 2014-10-10 Drill / Deepen / Reopen No Violations At 1800 hrs. arrived on location to perform the casing test with Halliburton and Company man Kevin Toothman. Halliburton loaded the Casing and took pressure up to 330 psi. Held for 30 minutes. Shut down release.
