Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1546444819 2019-01-02 Drill / Deepen / Reopen No Violations Status check inspection found a top hole auger rig owned by Lightning Energy on location and rigged up on the hole drilling a 36 diameter conductor well bore at 70. The borehole will be drilled to 100 and a 30 diameter conductor pipe set and cemented. Also noted is due to hole conditions a 42 diameter casing string had to be set to a depth of 31 from the bottom of the cellar floor which is 8 below grade level. The 42 was grouted from surface. The location is in an old surface mine area.
1546540825 2019-01-03 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job with Lightning Energys top hole rig and cement service. A 36 well bore was drilled to 110 with a previous string 42 in diameter set to 39 from ground level. With the previous string set the remainder of the well bore drilled dry with no water present prior to cementing. The top hole rig company set 110 of 30 diameter casing. The casing was cemented using a 103 barrel cement mix consisting of 490 sacks of class A cement with the cement additives being 3% calcium and 1/4 pound per sack loss circulation material. The cement was displaced with 65 barrels of fresh water with 14 barrels of cement returns witnessed to surface inside the cellar. There was 1 of cement left in the cellar floor to form a liquid tight seal.
1808607508 2018-12-28 Drill / Deepen / Reopen No Violations The said well was spudded by Lightning Energys auger rig on 12/21/2018. The rig drilled a 36 diameter conductor well bore to a measured depth of 62 from ground level. At that point the mine spoil back fill caused an excessive hole collapsing condition and the company representative Curtis Stevenson notified me they would spot cement over the unstable area and resume drilling after the McKeegan # 2 & # 4 wells conductor casing were set and cemented.
