0052556296 | 2019-03-15 | Casing String Witnessed | No Violations | On site inspection for a 8:00 am call out to witness the conductor casing cement job with a 3rd party top hole rig on location owned by Cellar Tech, also present id a cement service identified as Lightning Energy. The company has set a Cellar Tech Cellar the measured 9 deep and 7 x 10 square. The rig drilled a 40 diameter conductor borehole to a measured depth of 100 and set 100 of 30 .375 wall conductor. The conductor was cemented using 81 barrel 385 sack Class A cement mix with the cement weight verified at 15.6 pound per gallon by weight of scale.30% cement excess was ran. The cement was displaced with 73.5 barrels of fresh water. Cement was witnessed back to surface inside the cellar, the cement stayed up to surface. The company man called me out for the job at 8:00 am but it did not start until 14:00 pm. |

1554294569 | 2019-04-03 | Casing String Witnessed | No Violations | On site inspection accompanied by ODNR Inspector Ryan Armstrong to witness the cementing of the 18.625 diameter surface casing mine string with Patterson Drilling rig 292 on location along with Schlumberger Cement Service. Initially on 04/01/19 the casing was attempted to be ran and set down early at was believed to be on or near the bottom of the mine floor at 568. The company pulled the 18.625 and re drilled (reamed) the bottom section of the well bore. Casing was then re inserted into the well bore to a depth of 598 with the #8 Pittsburgh Coal seam encountered between the depths of 555 and 568 with all depths measured off a drilling rig floor heigth of 28 above ground level elevation, the total drilling depth reached was 618. Upon the landing of the casing a safety meeting was held and Schlumberger Cement Service commenced installing the cement head and flow lines and proceeded to initiate the casing cement job. Schlumberger cemented the casing by first pumping 200 barrels of fresh water followed by a 230 barrel cement mix consisting of 1,080 sacks of class A cement. The cement weight was verified by weight of scale at 15.6 pound per gallon before starting downhole with the cement mix. The cement yield was 1.2. Circulation was established during the pumping of the cement. 40 barrels of cement returns were witnessed to surface and the casing was pressure tested to 900 pounds pressure upon the landing of the wiper plug. |

1563680167 | 2019-04-24 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed Patterson rig UTI 292 was currently working on a gasket in the standpipe. Patterson 292 had drilled the 8.5-inch diameter lateral section of the well bore at 19,244 feet, there was a small 2-foot flare at the time of my inspection. After speaking with EQT consultant Kevin Slater he informed me that they intend to drill the 8.5-inch section of the well bore to an estimated total depth of 21,250 feet. I performed a walk around inspection on the rig and noticed there was some wood shavings on the ground around their mix off material. Kevin Slater said that he had containment contractors on the way to build a secondary containment around the materials. He also had a vacuum truck going around location and removing standing water, and once containment is completed will remove the mix off material from the ground. I informed Kevin to keep me updated on the progress of the well. |
1696572770 | 2019-04-01 | Casing String Witnessed | No Violations | On site inspection to witness the 18.625 surface mine string cement job with Patterson Drilling rig 292 on the hole and a casing crew identified as PTS, Schlumberger Cement Service is standing by to cement the casing. Upon arrival at 12:05 Pm the rig was running the 18.625 J-55 buttress thread casing with a proposed casing set depth of 598 and a well bore total drilling depth of 618 measured off of the rig floor. The #8 Pittsburg Coal came in at 555 to 568. The rig was unable to get casing past the mine floor and I left location at 6:50 pm. |
1716479001 | 2019-04-17 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed Patterson UTI 292 was currently rigging up bottom hole assembly in preparation to drill out the 9.625-inch diameter casing stage tool at 5,438 feet and the 9.625-inch diameter casing shoe track at 8,629 feet. After speaking with company man Kevin he informed me that after drilling out the stage tool and shoe track they will begin drilling the 8.5 inch diameter curve and lateral. Kevin informed me that they plan to drill to an estimated total depth of 21,203 feet. I performed a walk around inspection on the rig and did not see any issues that needed addressed. |
1784773263 | 2019-06-12 | Preliminary Restoration | No Violations | Status check inspection found a total of 5 wells present on the well pad. 3 were previously drilled and produced and 2 were recently drilled. The 2 recent wells have been drilled and are awaiting stimulation. Both wells were constructed and set inside a Cellar Tech cellar below ground level elevation. The well pad restoration meets ORC 1509.072 B. |
