At present there was no noted pressure on the gauge. They are monitoring the pressure daily. They are to notify myself or Northeast Regional Supervisor Tom Hill of any changes.
1417452076 2014-11-19 Drill / Deepen / Reopen No Violations On site with District Supervisor - Rick Anderson and Regional Supervisor - Tom Hill to conduct a status check on drilling operations related to the Nomac Drill Rig #77. Met with the company man on site to review the status of the drilling. At that time, the drilling had been stopped to conduct a drill depth survey which was at 6100 feet. The blow-out preventer was in place and in good condition. The permit was posted and test charts were in compliance and on site in the company man trailer. The spill prevention controls were in good condition and installed accordingly. Overall the site was in compliance. No violations were found.
1418654621 2014-11-05 Drill / Deepen / Reopen No Violations Tophole Drilling drilled a 26 hole to a depth of 120 and ran 120 of 20 94 #/ft. welded conductor casing. Express Energy Services pumped 9 barrels of fresh water ahead followed by 230 sacks of class A cement with 2% CaCl and .05 lb/gal of floseal. Displaced with 39 barrels of fresh water. 10 barrels of cement returned to surface.
1421338814 2014-12-23 Drill / Deepen / Reopen Violations Noted On December 22, 2014, DOGRM Inspector Robert Roush was on site to witness the installation and cementing of the surface casing at the CTF 32-13-4 #2H. While reviewing drilling information from prior wells on the CTF pad, Inspector Roush discovered that the surface casing for the first well drilled on the pad, the CTF 32-13--remedial at this time. Well construction will be further reviewed. The annulus between the surface casing and intermediate casing will need to be monitored for the life of the well. Further administrative action may be taken.
1421847184 2015-01-21 Administrative Inspection No Violations Per the Divisions request Chesapeake Exploration LLC reported annular pressure from between the surface casing and intermediate casing from 01/06/2015 to 01/20/2015. Reported pressure for the time period was zero. Operator is currently monitoring the pressure with a zero to 10,000psi gauge. Inspector John Fleming requested that a zero to 5,000psi gauge be placed on the annulus for monitoring.

1450440915 2015-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1454600226 2016-02-04 Administrative Inspection No Violations Chesapeake Exploration LLC submitted annulus readings from between the surface casing the intermediate casings on 1/22/2016. Annulus pressure continues to be zero. Annulus pressure monitoring started 05/07/2015 latest reading was taken 1/18/2016.
1469025583 2016-06-30 Production Wells No Violations I performed a Compliance Notice follow up # 1421338814. There is pressures gages installed on the 13-3/8 casing which has 0 pressure. There is 158 psi on the 9- 5/8 intermediate casing. The compliance notice can be closed at this time. The pressure gages will remain and be monitored at all times.

1646033663 2018-11-07 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 270 psi. I performed a walk-around inspection of the production pad and no violations were observed at the time of inspection.
1679395327 2018-04-23 Production Wells No Violations I conducted a status check of the pad. The pad is equipped with eight producing wells, and partially drilled wells with conductor casing (# 22624 & 22623). The surf/int pressure gauge displayed a reading 0 psi, and the int/prod displayed 251 psi. The tank battery consisted of six -400 bbl production tanks. Other equipment on the pad consisted of the following.




The access road was stable. No violations were observed at the time of inspection.
