0096733417 2016-11-03 Completion Testing No Violations I was on the Vaughn Pad to observe stimulation operations on the Vaughn # 1-H, Vaughn # 2-H, Vaughn #3-H and Vaughn #4-H. Go Frac supplied the pumping services, Renegade Wireline provided perforation and frac plug services. I observed the 2nd stage of the Vaughn # 2-H well. The second stage covered the interval from 15,270 MD to 15,408MD. There were five clusters of perforations per stage. Each cluster contained 12 shots ( 0.42 EHD ) at six (6) shots per foot with 60 degree phasing. The stage fluid volume was 8105 barrels, which transported 381,000 lbs. of sand. The stage treated at 65 bpm with a pressure of 7800 psi. Simultaneous operations were being conducted on the Vaughn # 3-H well by Renegade Wireline Services. A survey of the Vaughn Pad did not identify any violations.
1768475614 2018-06-20 Production Wells No Violations I conducted a status check of the well and production pad. The pad is equipped with four wells (permit numbers 2718, 2719, 2720, 2721) and this well is equipped with an open flow assembly. Proper identification was posted at the well and lease entrance. The tank battery is surrounded by a steel dike. The enclosed tank battery and contained less than one inch of standing water. I tested the standing water for chlorides using a low-range QuanTab, which concluded in a reading of under 33 pppm. The access road was stable and drivable. I performed an inspection of the perimeter of the pad, and erosional controls appeared to be in place and functioning.

I did not locate a surface-intermediate annular pressure gauge. I contacted operator who stated they in the process of acquiring gauges from the supplier, and an annular gauge will be installed as soon as possible.

2079099533 2016-12-02 Completion Testing No Violations I was on the Vaughn pad to observe the stimulation of # 2 - H well. The stage was the 119th out of a total of 158 stages for the pad. ProFrac was the frac contractor for the pad. As the stage was started the material Blender broke down and the operation stopped. Josh Kahle ( Rex Energy Completions Engineer ) informed me that the delay would be quite awhile to transport another Blender and exchange out. A walk around inspection covered the sand handling, wireline, brine off loading and chemical areas of the pad. No violations were identified during this inspection.
