0051163342 | 2018-04-02 | Drill / Deepen / Reopen | No Violations | I was on site to check the drilling status of the well. Patterson #570 was drilling at a depth of 10, 251 in the lateral section, which was landed at 8,042. I spoke to the consultant on site who stated that there have been no issues while drilling. I performed a walk-around inspection of the site and no violations were observed at the time of inspection. |

0865533692 | 2018-04-06 | Drill / Deepen / Reopen | No Violations | I was on location and talked with Brett Farnsworth Drilling Consultants for Chesapeake Energy about the cementing of the 13-3/8 surface casing coming up tonight or tomorrow. I walked the location and found no violations at this time. Permit is posted. They will call when ready to cement the surface. |
0898509519 | 2018-05-24 | Drill / Deepen / Reopen | No Violations | I was on site during preparations for the start of stimulation operations, which were anticipated to begin by 05/25/2018. I requested copies of the cement bond logs to review the cement top of the 5.5 casing. My interpretation indicated the top of good cement to be at ~3960. The depth of kick-off point was reported to be at 7,240. I performed a walk-around inspection of the site and no violations were observed at the time of inspection. |
1407445008 | 2018-05-22 | Drill / Deepen / Reopen | No Violations | I was on site while equipment was being moved on site for well stimulation. The wellhead was equipped with a 15k frac valve. I spoke to the Chesapeake representative on site and reviewed the permit and notifications. Completion operations are expected to commence on 05/24/2018. No violations were observed at the time of inspection. |
1523292062 | 2018-03-26 | Drill / Deepen / Reopen | No Violations | I met DOGRM Ashley Raff on location and we talked to Danny Lambert Drilling Consultants for Chesapeake Energy I will be helping to looking after the rig when Ashley is plugging the I&O well in her area. |

1912785874 | 2018-11-26 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. Three producing wells (22745, 22744, 22642) are located on the pad. This well is open flow with a wellhead Christmas tree assembly. The surface-intermediate annular pressure gauge displayed a reading of zero psi, and the intermediate-production gauge displayed zero psi. Jersey barriers were installed around the wellhead. The tank battery consisted of eight 400 BBL storage tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:
eight storage tanks; five labelled as containing condensate, and three as containing produced water one low-pressure vertical separator three sand separators three gas processing units one combuster unit one oil stabilization unit Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface and was in good repair. The lease road is stable and drivable. Preliminary restoration passed. |
2055944481 | 2018-03-26 | Drill / Deepen / Reopen | No Violations | I was on site to check the drilling status. Patterson #570 was preparing to spud the well after skidding from the #6H well. I spoke to the company man on site about the surface casing depth and upcoming cement job. I performed a walk-around inspection of the site and no violations were observed at the time of inspection. |
