1928766 Primary Facility 2010-11-18 Routine/Complete Inspection No Violations Noted At the time of this inspection, well drilled not yet equipped for production. Site properly signed with ES plan. Appears all bmps are installed and functioning according to plan. Access road stable. Recommend operator rock culvert outlets and temporarily seed/mulch prior to snowfall to help resist possible potential for erosion and ensure stabilization of disturbed areas. Two above grade pits located onsite. Left Mike Cioffi a message (11-18-10 @1230pm) to ensure structural integrity of pit liners are maintained, as well as freeboard, a section of liner appeared to come loose from wind. Freeboard ok. No accelerated erosion or sedimentation.
1929836 Primary Facility 2010-11-23 Drilling/Alteration No Violations Noted Well sign present. E&S controls were implemented. At the time of inspection, no E&S or IW issues. Both pits covered with 20 mil plastic and security fence. No freeboard issue. Falcon was contracted. Stratigraphy consisted of coal, ss and sh. Well completed and anticipated depth was 4970 ft. Well was hydro fractured and No signs of gas leak from around well and equipment.
1965927 Primary Facility 2011-04-13 Routine/Complete Inspection No Violations Noted At the time of inspection, site Id, ES plan and well tag present. Well drilled and in production. Brine tank surrounded by dike appears stable. Site has not yet been re-graded or seeded/mulched, until final permanent restoration is achieved operator should continue to monitor all temporary bmp¿s. Access road stable, although dept recommends that culvert outlets are rocked to reduce direct runoff into field to prevent a possible pollution event. No accelerated erosion or sedimentation issues were noted in relation to site or access road, although it is recommend that operator stabilize site to reduce the risk for a possible pollution event.
