2469214 Primary Facility 2016-03-25 Routine/Complete Inspection In Compliance with Policy At the time of inspection well appears equipped for production. Site permanently stabilized. No E&S issues in relation to site and/or access road. Well tag present and affixed in a permanent manner, however tag is illegible. During inspection Department observed a slow drip from the brine tank discharge valve. An approximate area measuring 2’x2’ directly below the leaking discharge valve appears to be impacted, as indicated by visible dampness and pooling of fluids. Department called Operator and left a message requesting responsible party to contact Department.

UPDATE At 12:15 Operator (Mr. Andron) returned Department’s call. Department informed Operator of above findings and recommended Operator improve tag legibility and perform cleanup in accordance with Spill Policy and follow-up with Department upon investigation of reported concerns. And if applicable, to quantify release and identify all materials released within 24 hours. SEE ADDITIONAL COMMENTS ON PAPER SAIRs
